Immunostimulants from naturopathy

Background

Many patients with a malignant disease want an accompanying naturopathic therapy whereby one of the main goals is to strengthen the immune system. Immunostimulants from naturopathy are available without a prescription and are taken by many patients in addition to the conventional oncological treatment.

Objective

The objective of our article is to present the data on immunostimulants from naturopathy with respect to myeloproliferative diseases.

Material and methods

The following article is a narrative review.

Results

Immunostimulants which have been shown to activate specific immune parameters include echinacin, umckaloabo, mistletoe and beta glucans from medicinal mushrooms. In addition, there are stimulants from the thymus and therefore of animal origin. Data from clinical studies on the use of naturopathic immunostimulants in myeloproliferative diseases are limited. Above all, there are no reliable data that the additional use of naturopathic immunostimulants could lead to an improvement of relevant clinical parameters. On the contrary, preclinical data show that a non-specific activation of the immune system often results in activation of malignant cells as well.

Conclusion

There is a potentially negative benefit-risk ratio. For this reason, usage of naturopathic immunostimulants in myeloproliferative cancers is prohibited. In the course of patient activation information about the importance of a healthy lifestyle, physical activity and a balanced diet is more appropriate.

Zusammenfassung

Hintergrund

Viele Patienten mit einer malignen Erkrankung wünschen sich eine begleitende naturheilkundliche Therapie. Ein Hauptziel ist hierbei die Stärkung des Immunsystems. Immunstimulanzien aus der Naturheilkunde sind rezeptfrei erhältlich und werden von vielen Patienten zusätzlich zu der konventionellen onkologischen Therapie eingenommen.

Ziel

Es erfolgt eine Darstellung von Immunstimulanzien aus der Naturheilkunde und deren Einsatz bei myeloproliferativen Erkrankungen.

Material und Methoden

Der Beitrag ist eine narrative Übersichtsarbeit.

Ergebnisse

Zu den Immunstimulanzien, die nachweislich zu einer Aktivierung von spezifischen Immunparametern führen, gehören Echinacin, Umckaloabo, Mistel und β‑Glucane aus sog. Heilpilzen. Zusätzlich gibt es Stimulanzien aus Thymus, also tierischen Ursprungs.

Die Datenlage aus klinischen Studien zum Einsatz von naturheilkundlichen Immunstimulanzien bei myeloproliverativen Erkrankungen ist limitiert. Vor allem gibt es keine belastbaren Daten, dass es zu einer Verbesserung von relevanten klinischen Parametern durch den zusätzlichen Einsatz von naturheilkundlichen Immunstimulanzien kommen könnte. Im Gegenteil zeigen v. a. präklinische Daten, dass durch eine unspezifische Aktivierung des Immunsystems häufig auch eine Aktivierung von malignen Zellen resultiert.

Diskussion

Es ergibt sich ein potenziell negatives Nutzen-Risiko-Verhältnis. Aus diesem Grund verbietet sich der Einsatz von naturheilkundlichen Immunstimulanzien bei myeloproliferativen Krebserkrankungen. Im Zug der Patientenaktivierung sind Informationen zur Bedeutung eines gesunden Lebensstils, zur körperlichen Aktivität und ausgewogenen Ernährung besser geeignet.
