Renal artery angioplasty

Description

Renal angioplasty was attempted on 38 renal arteries in 34 patients and was technically successful in 84.2%. Twenty-four patients were followed for 1 to 36 months (mean 15.3 months). The hypertension was cured or improved in 18 (75%), unchanged in 2 and recurrent after an initial period of improvement in four. Six patients were not available for further evaluation as five had additional renovascular surgery and one was lost to followup. Four patients had major complications; loss of the kidney occurred in one patient (2.9%). Respiratory renal mobility, predilatation with tapered teflon catheters and the contralateral femoral approach were found to be important technical aids for successful renal artery angioplasty. Abstract (German)

Bei 34 Patienten wurde eine Nierenarterienangioplastik an 38 Nierenarterien durchgefuehrt. In 84,2% der Faelle verlief der Eingriff erfolgreich. 24 Patienten wurden ueber einen Zeitraum von 1-36 Monaten nachuntersucht (mittlere Nachuntersuchungsdauer 15,3 Monate). Der Bluthochdruck wurde erfolgreich behandelt oder herabgesetzt bei 18 Patienten (75%), blieb unveraendert bei 2 Patienten und stieg nach anfaenglicher Besserung bei 4 Patienten wieder an. 6 Patienten konnten nicht nachuntersucht werden, da bei 5 Patienten zusaetzlich eine Nierengefaessoperation durchgefuehrt werden musste und ein Patient sich keiner weiteren Nachuntersuchung unterzog. Bei 4 Patienten traten groessere Komplikationen auf; Verlust der Niere trat bei einem Patienten ein (2,9%). Als wichtige technische Hilfsmittel bei einer erfolgreichen Nierenarterienangioplastik erwiesen sich: Vorerweiterung mit Hilfe sich verjuengender Teflonkatheter und ein kontralateraler femoraler Ansatz bei der Durchfuehrung der Angioplastik.
