Optimized myocardial scintigraphy using SPECT and quantitative evaluation

Description

Myocardial scintigraphy as a highly sensitive, non-invasive method diagnosis CHD forms the interface to invasive diagnosis procedures. A precise characterization of the method by itself as well as the possible gain of diagnostic information has to precede wide spread application. One problem exists in the determination of the specificity in a group of patients with a high prevalence of the disease. The introduction of quantitative parameters into the evaluation permits to critically analyze the criteria of decision by the receiver-operator characteristic and optimize sensitivity and specificity. The sensitivity was checked in 70 patients with confirmed CHD. The reference values for the quantitative parameters were derived from a group of patients with a low probability of CHD (< 3%). With this approach myocardial scintigraphy using SPECT and quantitative evaluation could be optimized for a sensitivity of 90% and a specificity of 93%. (orig.)

Abstract (German)

Die Myokardszintigraphie als sehr sensitives, nicht-invasives Verfahren in der Diagnostik der koronaren Herzkrankheit (KHK) steht an der Schnittstelle zur invasiven Diagnostik. Eine praezise Charakterisierung der Methode als solche, wie auch des erzielbaren Informationsgewinnes, muss einem breiten Einsatz vorausgehen. Ein Problem besteht in der Bestimmung der Spezifitaet in einem Patientengut hoher Praevalenz. Die Einfuehrung quantitativer Parameter in die Auswertung machen die Entscheidungskriterien einer Receiver-Operator Analyse zugaenglich und gestatten durch eine Optimierung die Definition von Sensitivitaet und Spezifitaet. Die Sensitivitaet wurde an 70 Patienten mit gesicherter KHK ueberprueft. Die Referenzwerte fuer die quantitativen Parameter wurden an einer Gruppe von Patienten mit einer niedrigen Wahrscheinlichkeit an einer KHK zu leiden (< 3%) gewonnen. Die quantitativ ausgewertete Myokardszintigraphie in SPECT Technik konnte so auf eine Sensitivitaet von 90% bei einer Spezifitaet von 93% optimiert werden. (orig.)
