18F-labeled fluoro-2-deoxyglucose positron emission tomography - computed tomography (18F FDG PET/CT) is a noninvasive molecular imaging test with in-built feature of whole-body survey, often useful in situations, where the diagnosis based on clinical findings, results of routine serological, radiological, and biochemical tests are difficult. We report a case of 59- year-old female presented with fever and neurological signs and symptoms. She underwent 18F FDG PET/CT scan, which showed interesting findings and utility in management of patient. The present study aims to evaluate the utility of 18F FDG PET/CT scan in diagnostic workup and treatment response evaluation in a case of neurosarcoidosis. In inflammatory of unknown origin presenting with neurological features, 18F FDG PET-CT can not only aid in knowing the extent of disease detecting any biopsiable site but also help in the evaluation of response
