A giant aneurysm of the ascending aorta accompanying type 2 dissection that detected incidentally: CT angiography findings

Full text: Background: An aneurysm is an abnormal weakened and bulging area in the wall of an artery. Aneurysms can form in any blood vessel, but they occur commonly in the aorta. A ruptured thoracic aortic aneurysm can cause life-threatening bleeding. Objective: We aim to present computed tomography angiography finding of a case with giant aneurysm of the ascending aorta accompanying type 2 dissection that detected incidentally. Materials and methods: 82-year-old female patient refer to cardiology polyclinic with dyspne. Ascending aorta aneurysm detected by echocardiography examination. Then the patient referred to Radiology department for CT angiography. Results: Aneurysm of tubular segment of ascending aorta reaches approximately 12x12 cm in size with the type II dissection were detected in CT angiography. Dissection flap does not include the aortic sinus of valsalva and ending just before braciosephalic trunk orifis. Intimal defect diameter was approximately 5 cm and include partial thrombosis in the false lumen. Effusion was detected in the pericardial and mediastinal recesses. Conclusion: Increase in the diameter of an aortic aneurysm, increasing risk of rupture and may lead to high rates of morbidity and mortality. MDCT angiography is fast, noninvasive and efficient imaging modality for detecting of aortic aneurysms and dissections
