Radiation therapy for patients of malignant salivary gland tumors with positive surgical margins

Description

Purpose: Surgery is an essential part of treatment for tumors of the salivary gland, yet there is increasing evidence in the literature supporting the necessity of adjuvant radiation therapy. The patients described in this report were selected to receive postoperative radiation therapy because they were found to have positive margins. A total of 17 patients with malignant tumors originating from the major salivary glands seen between 1970 and 1988 who were treated with surgery and postoperative radiation therapy were reviewed. All patients had positive surgical margins and the overall local control at five years was 65%. Classified by T-stage, local control was obtained in all two patients for T1 disease, in five of six for T2, in four of six for T3, and in none of three for T4. At five years, the ratio of patients free of distant metastases was two of two for T1 lesion, four of six for T2, three of six for T3, and none of three for T4. Five-year survival was obtained in all eight patients with T1 and T2 lesions, four of six for T3, and one of three for T4. Patients with neck nodal metastases present in the neck at admission did worse than those with negative nodes, with 0% (none of three patients) free of locoregional recurrence vs. 71% (eleven of 14), 0% (none of three) free of distant metastases vs. 63% (nine of 14), and 0% (none of three) survival at five years vs. 93% (13 of 14). It was concluded that postoperative radiation therapy for patients with positive surgical margins was effective for T1 and T2 disease. However, patients with T3 and T4 disease require more aggressive therapy. Patients with nodal metastases in the neck at admission tended to have distant metastases and had poor prognoses

Abstract (German)

Ziel: Die Operation kann als Therapie der Wahl fuer Tumoren der Speicheldruesen angesehen werden; in der Literatur finden sich jedoch zunehmend Hinweise, die die Notwendigkeit einer zusaetzlichen adjuvanten Strahlentherapie belegen. Die in diesem Bericht zusammengestellten Patienten wurden fuer eine postoperative Strahlentherapie ausgewaehlt, weil bei der histologischen Aufarbeitung die Schnittraender des Praeparates positiv waren. Die Daten von insgesamt 17 Patienten mit malignen Tumoren der Speicheldruesen, die zwischen 1970 und 1988 operiert und postoperativ bestrahlt worden waren, wurden analysiert. Bei all diesen Patienten waren am Absetzungsrand des Operationspraeparates Tumorzellen gefunden worden. Ergebnisse: Die lokale Kontrollrate im Gesamtkollektiv nach fuenf Jahren betrug 65%. In Abhaengigkeit vom T-Stadium wurde eine lokale Kontrolle bei den beiden Patienten mit T1-Tumor erzielt, bei fuenf von sechs Patienten mit T2-Tumoren, bei vier von sechs Patienten im Stadium T3 und bei keinem der drei Patienten im Stadium T4. Nach fuenf Jahren waren keine Metastasen nachweisbar bei zwei von zwei Patienten im Stadium T1, bei vier von sechs Patienten im Stadium T2, bei drei von sechs Patienten im Stadium T3 und bei keinem Patienten im Stadium T4. Fuenf Jahre ueberlebten alle acht Patienten mit T1- und T2-Tumoren, vier von sechs Patienten im Stadium T3 und einer von drei Patienten im Stadium T4. Patienten mit zervikalen Lymphknotenmetastasen hatten eine schlechtere Prognose als diejenigen mit negativem Lymphknotenstatus: 0% (keiner von drei Patienten) lokoregionale Rezidivfreiheit vs. 71% (elf von 14), 0% (keiner von drei) metasasenfrei vs. 63% (neun von 14) und 0% (keiner von drei) Fuenf-Jahres-Ueberleben vs. 93% (13 von 14). Schlussfolgerungen: Eine postoperative Bestrahlung bei Patienten mit positiven Schnittraendern war erfolgreich in den Stadien T1 und T2. Patienten im Stadium T3 und T4 benoetigten ein aggressiveres Vorgehen. Patienten mit zervikalen Lymphknotenmetastasen zum Zeitpunkt der Bestrahlung scheinen ein hohes Risiko fuer Fernmetastasen aufzuweisen und hatten eine schlechte Prognose
