Management of 80 Sinonasal Undifferentiated Carcinomas. Retrospective multicentre study of the French Network of Rare Head and Neck Cancers (REFCOR)

Objectives: Sinonasal undifferentiated carcinoma (SNUC) is a rare and aggressive disease requiring multimodal treatment, and multiple new entities once included in the spectrum of SNUC, such as SWI/SNF-deficient carcinomas, are emerging. We aimed to provide new data regarding the role of chemotherapy and surgery and the prognostic factors of disease-free survival. Methods: This study was based on data from the REFCOR database and included patients with SNUC treated with curative intent from 2007 to 2021 across 22 centres in France. We collected data on treatments, histologic and oncologic outcomes, toxicities and prognostic factors. Results: A total of 80 patients were included in the analysis. Among the entire cohort, the 5-year progression-free survival (PFS) and overall survival (OS) rates were 58% and 63%, respectively. Approximately 100% of the patients were treated with irradiation, 29% with surgery, 56% with neoadjuvant chemotherapy (82% had either a partial or a complete response) and 76% with chemoradiotherapy. No treatment modality was associated with a better OS or PFS, including surgery (p=0.34). There was a trend for a better PFS for the patients treated with chemotherapy (neoadjuvant or concurrent, p=0.062). The patients with SWI/SNF-deficient carcinomas had a significantly worse PFS (p=0.021) and OS (p=0.03) than the rest of the population. Conclusion: In the management of localised SNUC among all patients treated with irradiation, surgery yielded no benefit, whereas the addition of chemotherapy tended to improve disease-free survival. Abstract (French)

Objectifs: Le carcinome nasosinusien indifferencie est une maladie rare et agressive necessitant une prise en charge multimodale et de laquelle de nouvelles entites emergent tel que les carcinomes SWI/SNF deficients. Notre but est d'obtenir de nouvelles donnees en ce qui concerne le role de chaque modalite therapeutique et les facteurs pronostiques. Methodes: Nous avons conduit une etude multicentrique dans 22 centres en France sur les patients traites avec un objectif curatif pour un SNUC. Nous avons collecte les donnees sur les rapports histologiques, les traitements administres, les resultats oncologiques, les toxicites et les facteurs pronostics. Resultats: Entre 2007 et 2021, 80 patients ont ete inclus dans l'analyse. Pour l'ensemble de la cohorte, la survie sans recidive et la survie globale a 5 ans etaient de 58% et 63% respectivement. Tous les patients ont ete traite par radiotherapie, 29% par chirurgie, 56% par chimiotherapie neoadjuvante (82% ayant eu une reponse partielle ou complete) et 76% par chimioradiotherapie. Aucune modalite therapeutique n'etait associee a une amelioration de la SG ou de la SSR, dont la chirurgie (p=0,34). Il y avait une tendance a une amelioration de la SSR pour les patients traites par chimiotherapie (neoadjuvante ou concomitante, p=0,062). Les patients porteurs d'un carcinome SWI/SNF deficient avaient des resultats significativement moins bons que le reste de la population en termes de SSR (p=0,021) et de SG (p=0,03). Conclusion: Dans la prise en charge du SNUC localise, dans une cohorte entierement traitee par traitee par radiotherapie, il n'a pas ete retrouve de benefice a une chirurgie mais l'ajout d'une chimiotherapie a montre une tendance a ameliorer la survie sans maladie.
