Utilisation of tracer monoclonal antibodies for the immunoscintigraphic detection of human colorectal cancers

Description
Two monoclonal antibodies, 17-1A and 19-9, with recognized human gastrointestinal cancers in cell cultures, were labeled with iodine 131 for immunoscintigraphic application. With the intact 131I-17-1A antibody, 21 out of 35 (60%) primary or secondary colorectal cancer sites were visualized, whereas all 21 nonepitheliomatous colic cancer sites or noncolic cancer sites were negative. With F(ab')2 fragments of the 19-9 antibody, 18 out of 27 (67%) colorectal cancer sites were positive. With both radioantibodies, the bestly contrasted tumor images were late, 4 to 5 days after injection. A study with paired-label technique, associating a specific iodine-131-labeled antibody with a non-specific iodine-125-labeled immunoglobulin, demonstrated, that tumor uptake was indeed specific for the 17-1A or 19-9 antibody in tumor and normal colon fragments obtained during operations on 4 patients. A preliminary prospective study showed that only immunoscintigraphy was able to confirm and localize a recurrence of rectal cancer in one patient. A larger series will be necessary to validate the clinical benefit of the technique, as compared with the results of other diagnostic techniques, before immunoscintigraphy can be proposed for routine clinical use
Abstract (French)
Deux anticorps monoclonaux, 17-1A et 19-9 specifiques, en cultures cellulaires, de cancers gastro-intestinaux humains, ont ete marques a l'iode 131 pour une application immunoscintigraphique. Avec l'anticorps entier 131I 17-1A, 21 sites sur 35 de cancers colorectaux, primitifs ou secondaires, ont ete visualises (60%) alors que les 21 sites de cancers coliques non epitheliomateux ou de cancers non coliques ont ete tous negatifs. Avec les fragments F( ab')2 de l'anticorps 19-9, 18 sites sur 27 de cancers colorectaux ont ete positifs (67%). Avec les deux radioanticorps les images tumorales les mieux contrastees ont ete obtenues a des temps tardifs au-dela de 4 a 5 jours apres l'injection. Une etude en double marquage, associant un anticorps specifique marque a l'iode 131 et une immunoglobuline non specifique marquee a l'iode 125, a demontre, sur des fragments de tumeur et de colon normal preleves a l'intervention chez 4 malades, que la fixation tumorale etait bien specifique de l'anticorps 17-1A ou 19-9. Une etude prospective preliminaire a montre, chez une malade, que seule l'immunoscintigraphie avait pu confirmer et localiser une recurrence d'un cancer rectal. Cet interet clinique doit etre confirme sur une large echelle, par rapport aux resultats des autres techniques diagnostiques, avant que l'immunoscintigraphie soit proposee en routine cliniqueAdditional details
