Radiotherapy clinic experience in vaginal carcinoma

Description

Post-operative and definitive Radiation Therapy (PRT and DRT) treatment of VCF patients and analysis of signs: FIGO distribution, age 60 g, histological type, degree of malignancy (G), tumor location in the lower 1/3 of the vagina and type RT. Material and method: For the period 2007-2016 in the Radiotherapy Clinic there were treated 26 patients with an average age of 55.7 years (29-80 years). There are 25 patients with squamous cell carcinoma and 1 with adenocarcinoma. In the group with PRT there are 50% (13) patients - in 9 patients there is a dose of 50-54Gy for the small pelvis and vaginal area, at 1 with R1 an overdose of 60Gy. In 1 patient, pre- and seropositive RT to 60Gy was performed. The patients with DRT were 50% (13), with a dose of 60Gy, 1 of which was up to 70Gy due to R1 resection and 2 patients discontinued RT at 38Gy and 48Gy. In 6 of the patients with LV type are irradiated and infected. up to 40Gy due to the distal location of the tumor. Results: Deaths from KVL were detected in 46.2% (12/26) and 7.7% (2/26) of breast and lung carcinomas. Depending on the stage, the distribution of deaths is: stage I and stage II - 33.3% (4), 66.7% (8) in III and IV c, histology - 11 deaths with squamous cell carcinoma and 1 with adenocarcinoma. With regard to age 60 years, deaths are 33.3% (4) / 66.7% (8)> 60 years, respectively, compared to G1 / G2 / G3 respectively 0% / 8.4% / 75 %. According to the type of RT, the deaths from VC in PRT were found at 7.7% (2) and at DRT. 38.5 (10). Regardless of the RT performed, in the distal localization of the tumor of six patients the deaths were in 4 of them. Conclusion: The low degree of malignancy is the leading negative factor in the patients with VC, followed by an advanced disease in III and IV, age> 60 years, definitively RT, placement of the tumor in a distal third, regardless of the histological appearance.

Лечебни резултати при следоперативно и дефинитивно лъчелечение (СЛЛ и ДЛЛ) на болни с карцином на влагалището (КВл) и анализ на признаците: разпределение по FIGO, възраст 60 г, хистологичен вид, степен на малигненост (G), разположение на тумора в долна 1/3 на влагалището и вид ЛЛ. Материал и метод: За периода 2007-2016 г. в Клиниката по лъчелечение са лекувани 26 болни с КВл на средна възраст 55,7 г. (29-80 г). С плоскоклетъчен карцином са 25 болни и 1 с аденокарцином. В групата със СЛЛ са 50% (13) болни - при 9 болни е реализирана доза от 50-54Gy за областта на малкия таз и влагалището, при 1 с R1 е извършено свръхдозиране до 60Gy. При 1 болна е проведено пред- и слеоперативно ЛЛ до 60Gy. Болните с ДЛЛ са 50% (13), като е приложена доза от 60Gy, при 1 от тях - до 70Gy, поради R1 резекция и при 2 болни е прекъснато ЛЛ при доза 38Gy и 48Gy. При 6 от болните с давата вида ЛЛ са облъчени и ингв. л. в. до 40Gy поради дистално разположение на тумора. Резултати: Установени са умирания от КВл при 46,2% (12/26) и 7,7% (2/26) от карциноми на гърдата и бял дроб. В зависимост от стадия разпределението на умиранията е: в I и II стадий-33,3% (4), 66,7% (8) в III и IV ст., спрямо хистологията - 11 умирания с плоскоклетъчен карцином и 1 с аденокарцином. По отношение на възрастта 60 г. умиранията са съответно 33,3% (4)/66,7% (8) >60 г, спрямо G1/G2/G3 смъртността е съответно 0%/8,4%/75%. Според вида ЛЛ умиранията от КВл при СЛЛ се установява при 7,7% (2) и при ДЛЛ 38,5 (10). Независимо от проведеното ЛЛ, при дистална локализация на тумора на 6 болни умиранията са при 4 от тях. Извод: Ниската степен на малигненост е водещ негативен фактор при болните с КВл, следват авансирало заболяване във III и IV ст., възраст >60г., дефинитивно ЛЛ, разположение на тумора в дистална трета, без разлика от хистологичния вид.
