Radioimmunological determination of 5a-pregnane-3, 20-dione and progesterone in the serum in the case of abortus imminens in early pregnancy

The standardised method of radioimmunological hormone determination of the progesterone concentration in the mother's serum between the sixth and eighteenth week of pregnancy allows for a reliable prognosis in the case of abortus imminens (Knapstein, 1980). Permanently low or declining values with living fetuses indicate an unfavourable ending of the pregnancy. In this work it shall be clarified to what extent the two hormones, progesterone and 5a-DHP, can be used as prognostic parameters. The hormones were retrospectively determined in 83 women with normally progressing pregnancies and 57 women with abortus imminens symptomatology. In the women with abortus imminens symptomatology and with an abortion within a week the progesterone as well as the 5a-DHP median values lay statistically significantly below the values of the normal ones. In the corresponding women with premature births, respectively full term births, the values do not differentiate so clearly. In the abortus imminens group 61% of the women showed progesterone values which lay below the 10th percentile of the normal group, 44% aborted within a week, 10% after a several week long latency, 5% carried full term. 46% of the women had 5a-DHP values which lay below the 10th percentile of the normal group, 35% aborted within a week, 7% after a several week long latency, there was one premature birth and one full term birth. 30% of the women had progesterone values which lay within the 80th interpercentile level of the normal group, 5% aborted within a week. 28% of the women had 5a-DHP values within this same level and 14% aborted within a week. In the normal and abortus imminens groups there is an association between serum progesterone and 5a-MHP. Specificity: 95% for progesterone and 5a-DHP. Die standardisierte Methodik der radioimmunologischen Hormonbestimmung der Progesteronkonzentration im muetterlichen Serum zwischen der 6. und 18. Schwangerschaftswoche erlaubt eine zuverlaessige Prognose beim Abortus imminens (Knapstein, 1980). Permanent niedrige oder abfallende Werte bei noch lebenden Feten zeigen einen unguenstigen Ausgang der Schwangerschaft an. In der vorliegenden Arbeit soll geklaert werden, inwieweit die beiden Hormone Progesteron und 5a-DHP als prognostische Parameter verwendet werden koennen. Die Hormone wurden retrospektiv bei 83 Frauen mit normal verlaufender Schwangerschaft und 57 Frauen mit Abortus imminens-Symptomatik radioimmunologisch bestimmt. Bei den Frauen mit Abortus imminens-Symptomatik und einem Abort innerhalb einer Woche lagen sowohl Progesteron- als auch 5a-DHP-Medianwert statistisch signifikant unter denen des Normalkollektivs. Bei den entsprechenden Frauen mit Fruehgeburt bzw. normaler Geburt zum Termin unterschieden sich die Werte nicht so deutlich voneinander. Im Abortus imminens-Kollektiv wiesen 61% der Frauen Progesteronwerte auf, die unterhalb der 10%-Perzentile des Normalkollektivs lagen. Es abortierten 44% innerhalb einer Woche, 10% nach mehreren Wochen Latenz, bei 5% normale Geburt zum Termin. 46% der Frauen hatten 5a-DHP-Werte, die unterhalb des Normalkollektivs-10%-Perzentile lagen. 35% abortierten innerhalb einer Woche, 7% nach mehreren Wochen Latenz, eine Frueh-, eine Normalgeburt. Innerhalb der 80%-Interperzentilebene des Normalkollektivs lagen 30% (Progesteron), 5% Aborte innerhalb einer Woche; 28% (5a-DHP), 14% Aborte innerhalb einer Woche. Im Normal- und Abortus-imminens-Kollektiv bestand eine Assoziation zwischen Serumprogesteron und -5a-DHP.
