Supportive and complementary treatment of fatigue

Background

Fatigue is one of the most common side effects in malignant diseases. The causes, duration and intensity of fatigue are complex.

Objective

Presentation of recent findings from evidence-based medicine for supportive and complementary treatment of fatigue in patients with hemato-oncological malignancies.

Methods

A selective literature review was carried out and a narrative summary of current evidence for supportive and complementary treatment of fatigue is presented.

Results

Prior to the initiation of a specific treatment, differential diagnoses should be excluded. Drug interventions are possible; however, physical activity is more suitable. This can be done during adjuvant therapy as well as after completion of treatment. In addition, a healthy diet helps to improve fatigue. Whether psycho-oncological interventions are helpful cannot be clearly answered on the basis of the available data. From the complementary field only selenium was found to have a positive influence on fatigue. All other holistic or substance-bound methods studied as a rule showed contradictory results.

Conclusion

Because fatigue affects many domains of health-related quality of life, treatment should always follow a multimodal approach after excluding differential diagnoses. An integral component should be physical activity. With appropriate adherence this shows the most sustainable success in the treatment of fatigue even after high-dose chemotherapy with stem cell transplantation and also has many other positive effects on the quality of life of patients. A healthy diet supports the result.

Zusammenfassung

Hintergrund

Fatigue ist eine der häufigsten Nebenwirkungen bei onkologischen Erkrankungen. Dabei sind Ursachen, Dauer und Intensität von Fatigue vielschichtig.

Ziel

Aktuelle Erkenntnisse aus der evidenzbasierten Medizin für die supportive und komplementäre Behandlung von Fatigue bei Patienten mit hämatoonkologischen Malignomen werden dargestellt.

Material und Methoden

Es wurde eine selektive Literaturrecherche und eine narrative Zusammenfassung derzeitiger Evidenz für die supportive und komplementäre Behandlung von Fatigue durchgeführt.

Ergebnisse

Vor Einleitung einer spezifischen Therapie sind die Differenzialdiagnosen auszuschließen. Medikamentöse Interventionen sind möglich. Besser geeignet ist allerdings körperliche Aktivität. Diese kann sowohl während der adjuvanten Therapie als auch nach Abschluss der Behandlung erfolgen. Auch eine gesunde Ernährung trägt zur Verbesserung von Fatigue bei. Ob psychoonkologische Interventionen hilfreich sind, lässt sich anhand der Datenlage nicht eindeutig beantworten. Aus dem komplementären Bereich konnte nur für Selen ein positiver Einfluss auf Fatigue nachgewiesen werden. Alle anderen bislang untersuchten ganzheitlichen oder substanzgebundenen Verfahren zeigten in der Regel widersprüchliche Ergebnisse.

Schlussfolgerung

Da Fatigue viele Domänen der gesundheitsbezogenen Lebensqualität beeinflusst, sollte die Behandlung nach Ausschluss von Differenzialdiagnosen grundsätzlich einen multimodalen Ansatz verfolgen. Integraler Bestandteil sollte dabei körperliche Aktivität sein. Diese zeigt, selbst nach Hochdosischemotherapie mit Stammzelltransplantation, bei entsprechender Adhärenz die nachhaltigsten Erfolge bei der Behandlung von Fatigue und hat darüber hinaus viele weitere positive Effekte auf die Lebensqualität der Patienten. Eine gesunde Ernährung unterstützt das Ergebnis.
