Effects of radiographic contrast media on cellular electrophysiology in the beating heart

Description

Electrophysiological effects of intracoronarily administered contrast media have been documented in 12 thoracotomized dogs at the cellular level by use of a modified microelectrode technique. Injections (n = 63) of 4 different contrast media uniformly led to a temporary cellular hyperpolarisation of the resting potential and prolongation of the action potential. Additional experiments with intracoronary injections of several electrolyte concentrations, mainly by a local deficiency of potassium ions and an excess of sodium ions. The significance of the findings for mechanisms underlying ECG-changes and ventricular arrhythmia by radiographic contrasts media will be discussed. (orig.)

Abstract (German)

Bei 12 thorakotomierten Schaeferhunden wurden die elektrophysiologischen Wirkungen verschiedener Roentgenkontrastmittel bei selektiver Koronarangiographie mit einer modifizierten Mikroelektrodentechnik untersucht, die die Ableitung zellulaerer Membranpotentiale am schlagenden Herzen in situ ermoeglicht. Die Auswertung von 63 Roentgenkontrastmittelinjektionen ergab bei Arbeitsmyokardzellen im Versorgungsbereich des betreffenden Gefaesses eine prompte und in Abhaengigkeit vom verwandten Kontrastmittel unterschiedlich ausgepraegte Hyperpolarisation des Ruhepotentials und Verlaengerung der Aktionspotentialdauer. Zusaetzliche Untersuchungen mit Modelloesungen ergaben, dass diese Befunde hauptsaechlich auf kontrastmittelbedingte Aenderungen des extrazellulaeren Elektrolytmilieus i.S. einer lokalen Hypokaliaemie und relativen Hypernatriaemie zurueckzufuehren sind. Die Bedeutung der Befunde hinsichtlich der EKG-Veraenderung und Arrhythmieausloesung durch Roentgenkontrastmittel wird dikutiert. (orig.)
