Results of simultaneous radio-chemotherapy in advanced head and neck cancer

Description
Patients suffering from locally advanced squamous cell carcinoma of the head and neck were treated with three courses of simultaneous radio-chemotherapy. Chemotherapy consisted of cis-platinum, 60 mg/m2 i.v. on day 2; 5-FU, 350 mg/m2 i.v. bolus on day 2; leucovorin calcium, 50 mg/m2 i.v. on day 2; 5-FU, 350 mg/m2/24 hrs continuously infused over 96 hrs from day 2 to day 5 and leucovorin calcium, 100 mg/m2/24 hrs continuously infused over 96 hours from day 2 to day 5 each course. Radiotherapy was administered from day 3 to day 11. 23,4 Gy were given in 13 fractions, twice a day with a minimum interval of four hours. This schedule was repeated on days 22 and 44. The total radiation dose amounted to 70,2 Gy/51 days. From 1984 to 1986, 62 patients were entered in this prospective trial. Three patients deceased due to massive hemorrhage during therapy, one patient was not eligible due to a second malignancy. 5/58 evaluable patients had a UICC-Stage III cancer, 53/58 had a UICC-Stage IV cancer. 48/58 (81%) showed a clinically complete response to therapy. 10/58 (17%) achieved partial responce three months after the end of treatment. In 16/58 patients loco-regional cancer was not controlled (minimum follow-up 2 years), in 12/58 distant metastases occurred. Loco-regional control rate is estimated at 66%±7% (Kaplan Maier). (orig.)

Patienten mit loko-regionaer weit fortgeschrittenen Plattenepithelkarzinomen im Kopf-Hals-Bereich wurden mit drei Zyklen simultaner Radio-Chemotherapie behandelt. Am Tag 2 wurden cis-Platin, 60 mg/qm i.v. als Kurzinfusion, 5-Fluorouracil (-FU) 350 mg/qm und Folinsaeure 50 (LV) mg/qm als i.v. Bolusinjektion appliziert, und von Tag 2 bis Tag 5 5-Fu, 350 mg/qm/24 h und LV 100 mg/qm/24 h ueber 96 h infundiert. Von Tag 3 bis Tag 11 wurden der Primaertumor und manifeste Lymphknotenmetastasen mit 23,4 Gy Zielvolumendosis bestrahlt, aufgeteilt zu 13 Fraktionen von je 1,8 Gy und 2 Fraktionen pro Tag mit einem minimalen Intervall von 4 Stunden. Der Behandlungszyklus wurde an den Tagen 22 und 44 wiederholt. Die Gesamtdosis betrug 70,2 Gy/51 Tagen. Zwischen 1984 und 1986 wurden 62 Patienten behandelt. Drei Patienten verstarben unter Therapie an Tumorarrosionsblutungen, ein Patient war nicht auswertbar. Von 58 (UICC-Stadium III:5, IV:53) auswertbaren Patienten wiesen 48 (83%) drei Monate nach Therapieende klinisch eine komplette Remission auf, bei 10/58 (17%) wurde eine klinisch partielle Remission erzielt. Nach einer minimalen Nachbeobachtungszeit von mehr als 2 Jahren trat bei 16/58 Patienten ein loko-regionaeres Rezidiv und bei 12/58 eine Fernmetastasierung auf, 23 Patienten leben tumorfrei, 8 sind tumorfrei verstorben. Nach 3 Jahren betraegt die loko-regionaere Tumorkontrollwahrscheinlichkeit 66%, die Wahrscheinlichkeit tumorfreien Ueberlebens 51% (Kaplan-Meier-Methode). Dieses Therapieprotokoll senkt die Rate an loko-regionaeren Rezidiven von mehr als der Haelfte nach alleiniger hochdosierter Bestrahlung auf weniger als 30%, waehrend die Haeufigkeit von Fernmetastasen nicht beeinflusst wird.
