Comparison of the planned thyroid hormone dose in the radioiodine test to the achieved thyroid hormone dose in the radioiodine therapy

The aim of the work was to review the previous dosage concepts of radioiodine therapy of benign thyroid diseases on the basis of an own cohort with 2768 patients. Two of the three dose concepts could be confirmed within the framework of this study. Only the dose concept of the therapy of focal autonomy with a planned thyroid herd dose of 350 Gy must be critically evaluated again. In a group of 552 patients, the achieved thyroid herd dose deviated highly significantly with - 27.4 % from the planned thyroid herd dose. Nevertheless, in a group of 58 patients connected to our own outpatient clinic, it could be shown in the follow-up after 6 months that the autonomy could be treated successfully despite the deviation of the thyroid herd dose. The volume of the adenoma as well as the Tc uptake in the thyroid scintigraphy decreased significantly after 6 months, the TSH level in the serum normalised. In conclusion, this means that a lower thyroid herd dose of 250 Gy would probably be sufficient to treat focal autonomy. However, the influences of prethyroidal attenuation, absorption and lower maximum uptake of radioiodine during therapy would have to be included in the calculation so that the planned thyroid herd dose is actually achieved.

Abstract (German)

Ziel der Arbeit war, die bisherigen Dosiskonzepte der Radioiodtherapie benigner Schilddrüsenerkrankungen anhand einer eigenen Kohorte mit 2768 Patienten zu überprüfen. Zwei der drei Dosiskonzepte konnten im Rahmen dieser Untersuchung bestätigt werden. Einzig das Dosiskonzept der Therapie der fokalen Autonomie mit einer geplanten Schilddrüsenherddosis von 350 Gy muss erneut kritisch evaluiert werden. In einer Gruppe von 552 Patienten wich die erreichte Schilddrüsenherddosis hoch signifikant mit - 27,4 % von der geplanten Schilddrüsenherddosis ab. Dennoch konnte in einer an die eigene Ambulanz angebunden Gruppe von 58 Patienten im Follow Up nach 6 Monaten aufgezeigt werden, dass trotz Abweichung der Schilddrüsenherddosis die Autonomie erfolgreich behandelt werden konnte. Das Volumen des Adenoms sowie der Tc-Uptake in der Schilddrüsenszintigraphie gingen nach 6 Monaten deutlich zurück, der TSH-Spiegel im Serum normalisierte sich. Schlussfolgernd heißt dies, dass eine niedrigere Schilddrüsenherddosis von 250 Gy wahrscheinlich ausreichen würde, um eine fokale Autonomie zu behandeln. Allerdings müsste man die Einflüsse der präthyreoidealen Abschwächung, der Absorption und des niedrigeren maximalen Uptake des Radioiods während der Therapie mit in die Berechnung einfließen lassen, sodass die geplante Schilddrüsenherddosis auch tatsächlich erreicht wird.
