Erratum: perturbative QED and QCD at finite temperatures and densities

This note lists a number of errors appearing in the equations presented in the paper Perturbative QED and QCD at Finite Temperatures and Densities. The paper originally appeared in a 1985 issue of this journal. The corrections to those equations are given. The errors are typographic and do not alter any conclusions
