Image guided radiotherapy: equipment specifications and performance - an analysis of the dosimetric consequences of anatomic variations during head-and-neck radiotherapy treatment

Description
Anatomic variations during head-and-neck radiotherapy treatment may compromise the delivery of the planned dose distribution, particularly in the case of IMRT treatments. The aim of this thesis was to establish 'dosimetric indicators' to identify patients who delivered dose deviates from the planned dose, to allow an eventual re-optimisation of the patient's dosimetry, if necessary, during the course of their radiotherapy treatment. These anatomic variations were monitored by regular acquisition of 3D patient images using an onboard imaging system, for which a rigorous quality control program was implemented. The patient dose distribution analysis and comparison was performed using a modified gamma index technique which was named gammaLSC3D. This improved gamma index technique quantified and identified the location of changes in the dose distribution in a stack of 2D images, with particular reference to the target volume (PTV) or organs at risk (parotids). The changes observed in the dose distribution for the PTV or parotids were then analysed and presented in the form of gamma-volume histograms in order to facilitate the follow up of dosimetric changes during the radiotherapy treatment. This analysis method has been automated, and is applicable in clinical routine to follow dose variations during head and neck radiotherapy treatment. (author)

Les variations anatomiques au cours de traitement ORL compromettent la bonne delivrance de la distribution de dose planifiee. Nous avons defini des indicateurs permettant d'identifier les patients pour lesquels la distribution de dose delivree s'ecarte de la distribution de dose planifiee. Ce travail a ete realise a partir de l'acquisition reguliere d'images 3D de patients issues d'un systeme d'imagerie embarque pour lequel nous avons etabli un programme d'assurance qualite. Nous avons ensuite developpe une analyse modifiee de l'index gamma, quantifiant et localisant l'ecart entre la distribution de dose delivree et celle planifiee, permettant de signaler la necessite de replanifier la dosimetrie au cours des seances d'irradiation; nous avons automatise cette methode, applicable en routine clinique. Nous avons par ailleurs determine des variations de distances et de volumes au sein du patient permettant de rendre compte des modifications des histogrammes dose-volume au cours du traitement.
