Radiologic manifestation of histiocytic lymphoma in the skeletal and central nervous system

Description

The radiographic manifestation of histiocytic lymphoma in the skeletal and central nervous system of 112 non-selected patients were reviewed in a retrospective study. Histiocytic lymphoma metastatic to bone and brain are late manifestations of the disease occurring in 15 and 4%, respectively. Involvement of the spinal cord and nerve roots is found in 6% and represents often the initial manifestation of the disease. Histiocytic lymphoma of brain presents angiographically as avascular mass without site predilection and on contrast enhanced computer tomography as hyperdense lesion without necrotic center. On myelography an extradural mass or complete block is found. The most common skeletal locations are spine, pelvis, rib and femur. The ratio of multifocal to unifocal skeletal manifestations is approximately 2 to 1. The radiographic appearance varies from completely lytic, often associated with a soft tissue mass, to osteoblastic, often with Pagetoid features. Early recognition of histiocytic lymphoma manifestations is important to guarantee prompt institution of appropriate therapy since the disease can progress extremely rapidly. (orig.)

Abstract (German)

Die roentgenologische Manifestierung histiozytischer Lymphome im Skelettsystem und ZNS von 112 willkuerlich ausgewaehlten Patienten wurden in einer retrospektiven Studie untersucht. Histiozytische Lymphome, die in den Knochen und im Gehirn metastasieren, sind Spaetfolgen, die bei 15 bzw. 4% der Patienten auftreten. In 6% der Faelle sind Rueckenmark und Nervenwurzeln mitbetroffen; in vielen Faellen ist dies eine Frueherscheinung der Erkrankung. Angiographisch erweist sich das histiozytische Gehirnlymphom als avaskulaere Masse ohne Bevorzugung einer bestimmten Lokalisierung; die kontrastverstaerkte Computertomographie zeigt eine stark verdichtete Laesion ohne nekrotisches Zentrum. Die Myelographie zeigt eine extradurale Masse oder einen geschlossenen Block. Im Skelett findet sich das Lymphom am haeufigsten an der Wirbelsaeule, im Becken, an den Rippen und an der Huefte. Das Verhaeltnis der multifokalen zu den unifokalen Manifestierungen im Skelett ist etwa 2:1. Roentgenographisch variiert das Erscheinungsbild zwischen vollstaendig lytisch, oft mit weicher Gewebsmasse, und osteoblastisch, oft mit den Zuegen der Pagetschen Krankheit. Die Frueherkennung der Manifestierungen des histiozytischen Lymphoms ist wichtig zur rechtzeitigen Einleitung geeigneter Behandlungsmassnahmen, da sich die Erkrankung ausserordentlich rasch ausbreiten kann.
