Implementation and interest of a recording system for (nearly-) incidents in a radiotherapy department

Description
Within the frame of an ISO 9002 certification, a system recording (nearly-) incidents has been implemented in a radiotherapy department. The authors report the results obtained after six years in terms of number of incidents, type of detection (by operators, physicians, and radiotherapists), time of detection (before, during or after the treatment), and impact on patient treatment.
