Hepatic pseudolesion after biopsy

Description

We report the diagnosis and differential diagnosis of a hepatic pseudolesion due to percutaneous liver biopsy in a 30-year-old female patient with known chronic hepatitis C and renal insufficiency. In the course of transplant preparation, an abdominal spiral-CT examination pre and post i.v.-contrast injection as well as an angiography with CT-hepaticography and CT-portography were performed. In these examinations a 1 cm, hepatocellular-carcinoma mimicking liver lesion was found; it was hypodense in the CT-portography and showed a marked enhancement in the CT-hepaticography. This 'pseudolesion', which was supposed to be due to the liver biopsy, resolved spontaneously. (orig.)

Abstract (German)

Im folgenden wird ueber das Bild sowie die Differentialdiagnose einer durch Leberpunktion hervorgerufenen 'Pseudolaesion' bei einer 30jaehrigen Patientin berichtet. Im Rahmen der Transplantationsvorbereitung bei bekannter Niereninsuffizienz und chronischer Hepatitis C-Infektion wurde eine Spiral-CT-Untersuchung des Abdomens nativ und nach intravenoeser Kontrastmittelinjektion sowie eine Angiographie mit CT-Hepatikographie und CT-Portographie durchgefuehrt. In diesen Untersuchungen fand sich ein etwa 1 cm grosser HCC-suspekter Leberherd, der in der arteriellen Phase ein deutliches Enhancement zeigte, sich in der CT-Portographie dagegen hypodens darstellte. Erst im nachhinein konnte eruiert werden, dass es sich hierbei um eine 'Pseudolaesion' handelte, bedingt durch eine Leberpunktion. Ohne entsprechende Therapie bildete sich dieser wieder zurueck.
