Value of computed tomography in histiocytosis X of the base of the skull in children

Description

Five cases of histiocytosis X involving the base of the skull in children are reported two were inaugural and three occured in the course of known histiocytosis. The authors emphasize the very important anatomo-clinical dissociation between poor clinical findings and very extensive anatomical lesions. Antero-posterior and lateral plain film of the skull may misjudge such lesions and are inadequate. CT plays a dominant role in this diagnosis because of the ability to identify bone destruction, soft tissue involvement and intracranial histiocytosis more accurately. The authors try to define the specific CT signs of the principal lesions encountered

Abstract (French)

Les auteurs rapportent cinq observations d'atteinte histiocytaire de la base du crane chez l'enfant, dont deux inaugurales et trois survenant au cours d'une histiocytose connue. Ils insistent sur la dissociation anatomo-clinique entre une symptomatologie fruste ou absente, et des lesions anatomiques tres etendues. Ils soulignent l'insuffisance du cliche simple du crane de face et de profil pour le depistage de ces anomalies. Ils preconisent un examen tomodensitometrique qui permet de mieux analyser les lesions osseuses. L'examen tomodensitometrique permet d'apprecier l'extension tissulaire intra et extra-cranienne et de rechercher des localisations intracerebrales, en particulier pituitaires. Ils essaient de degager une semeiologie des principales lesions rencontrees
