Risk of vertebral fracture after menopause: detection of high risk subjects by dual photon absorptiometry

Description

Dual photon absorptiometry of bone is used to detect in an exposed population those subjects who are high risk of fracture and also to follow up the evolution of these patients. 37 women who have suffered fractures have been compared to 41 women without fractures of similar age distribution. A highly significant correlation between body height and Bone Mineral Content (BMC) of the lumbar spine is found in the control group thus allowing the calculation of the expected BMC value for each patient. A crushing index is defined as the ratio of the observed BMC value to the expected one. Using this index, instead of the two more usual modes of BMC expression, leads to an improvement of the predictive estimation of fracture risk. The predictive value of such indices should still be improved. With this aim, further determinations of indices are desirable. The following requirements should be borne in mind: the physical data should be easily obtainable e.g. body height and weight and the meaning of the index based on these easily verifiable factors they should be easily understood

Abstract (French)

La densitometrie osseuse biphotonique est utilisee pour depister les sujets a risque fracturaire eleve au sein d'une population exposee et pour suivre l'evolution de ces patients. 37 femmes ayant des antecedents fracturaires ont ete comparees par cette methode a 41 femmes temoins de meme age. Le Contenu Mineral Osseux du rachis lombaire (CMO) des temoins est lie tres significativement a la taille, ce qui permet de calculer un CMO theorique. On definit un index fracturaire comme le rapport CMO observe/CMO theorique. La valeur predictive de l'index fracturaire s'est averee significativement superieure a celle des 2 modes d'expressions du CMO les plus utilises. Il apparait souhaitable d'ameliorer la valeur predictive de tels index en respectant 2 exigences: 1) Utiliser des renseignements quantifies simples (la taille, par exemple). 2) Garder a ces index un sens physique accessible a tous
