Evaluation of exposure of the eye lens and extremities to ionizing radiations among interventional physicians at Rouen University Hospital

Background: The number of interventional medical procedures performed in France is increasing every year. X-rays are used to guide the surgical procedure, but lead to secondary exposure of anatomical areas not protected by leaded equipment. The association between chronic occupational exposure to low doses of ionizing radiation and potential health effects in medical staff has been demonstrated: radiation-induced cataracts, subungual microvascular anomalies. The main aim of this study was to assess exposure of the eye lens and hands to ionizing radiation among interventional practitioners at Rouen University Hospital. Materials and methods: The study was observational, prospective and monocentric. All practitioners working in the interventional radiology department at Rouen University Hospital were included in the study from January 1, 2020 to June 30, 2020: four radiologists, four vascular surgeons, two interns and one radiology reference technician. The type of interventional activity performed was defined and quantified for each doctor, based on the dose-area product of all operations carried out during the follow-up period. Individual eye lens and ring dosimetry data were collected over a six-month period. In order to assess exposure of the eye lens and hands over twelve consecutive months, an annual extrapolation of eye lens and hand dosimetry was carried out. Results: The annual extrapolation of eye lens dosimetry showed that three radiologists in the interventional radiology department exceeded the exposure limit value (ELV), set at 20 mSv over twelve consecutive months from July 1, 2023, with values of 23.3, 27 and 33 mSv respectively. With regard to hand exposure to ionizing radiation, none of the practitioners exceeded the ELV of 500 mSv over twelve months for category A workers. However, one radiologist had an annual ring dose interval above the category B ELV, defined at 150 mSv. Conclusion: Interventional radiologists are repeatedly exposed to low doses of ionizing radiation over the course of their professional careers, which can lead to exposure limits being exceeded, particularly for the eye lens. All collective and individual protective equipment must be used to reduce this exposure. Appropriate dosimetric monitoring of the eye lens and hands is essential to quantify the exposure of these practitioners to ionizing radiation. The occupational physician must implement reinforced medical surveillance to prevent and detect any illness likely to be related to exposure to ionizing radiations

Contexte: Le nombre d'actes realises en medecine interventionnelle augmente chaque annee en France. L'utilisation des rayons X permet de guider le geste operatoire mais entraine une exposition secondaire des zones anatomiques non protegees par un equipement plombe chez les praticiens. L'association entre l'exposition professionnelle chronique a de faibles doses de rayonnements ionisants et de potentiels effets sur la sante du personnel medical a ete demontree: cataractes radio-induites, anomalies microvasculaires sous-ungueales. Le principal objectif de cette etude a ete d'evaluer l'exposition du cristallin et des mains aux rayonnements ionisants chez les praticiens interventionnels du CHU de Rouen. Materiels et methodes: Il s'agit d'une etude observationnelle, prospective et monocentrique. L'ensemble des praticiens intervenant dans le service de radiologie interventionnelle du CHU de Rouen a ete inclus dans l'etude du 1er janvier 2020 au 30 juin 2020: quatre radiologues, quatre chirurgiens vasculaires, deux internes ainsi qu'un des manipulateurs referents en radiologie. La typologie de l'activite interventionnelle exercee a ete definie et quantifiee pour chaque medecin en se basant sur le produit dose.surface de l'ensemble des operations realisees au cours du suivi. Un recueil dosimetrique cristallin et bague individuel a ete mis en place pendant une periode de six mois. Afin d'evaluer l'exposition du cristallin et des mains sur douze mois consecutifs, une extrapolation annuelle des dosimetries cristallin et mains a ete realisee. Resultats: L'extrapolation annuelle de la dosimetrie cristallin a mis en evidence un depassement de la valeur limite d'exposition (VLE), fixee a partir du 1er juillet 2023 a 20 mSv sur douze mois consecutifs, pour trois radiologues du service de radiologie interventionnelle, respectivement 23,3, 27 et 33 mSv. Concernant l'exposition des mains aux rayonnements ionisants, aucun praticien n'a depasse la VLE de 500 mSv sur douze mois pour les travailleurs classes en categorie A. Cependant, un des radiologues a presente un intervalle de doses bague annuelle au-dessus de la VLE de la categorie B, definie a 150 mSv. Conclusion: Les praticiens interventionnels sont exposes a de faibles doses de rayonnements ionisants de facon repetee au cours de leurs carrieres professionnelles, pouvant entrainer un depassement des valeurs limites d'exposition notamment au niveau du cristallin. L'utilisation de l'ensemble des equipements de protection collective et individuelle est necessaire pour reduire cette exposition. Un suivi dosimetrique adapte du cristallin et des mains est essentiel pour quantifier l'exposition de ces praticiens aux rayonnements ionisants. Le medecin du travail doit mettre en oeuvre une surveillance medicale renforcee afin de prevenir et depister toute affection susceptible d'etre en relation avec une exposition aux rayonnements ionisants.
