The ladder of the nuclear-medical thyroid diagnostics

Description

Diagnosing diseases of the thyroid requires an exact case history, the special physical examination of the patient, and the application of suitable technical examination methods. The individual methods of clarifying an abnormal thyroid function should be chosen carefully, according to the clinical questions, they should not be applied according to a scheme. The determining of thyroxine and/or triiodothyronine in the serum, the measurement of the binding capacity for thyroid hormones or of the free thyroid hormones and the TRH-test have almost totally replaced the 131I-Radioiodine-two phase test, especially since the (gamma camera-)scintiscanning of the thyroid with sup(99m)Tc-pertechnetate or 123I-iodide can be combined with storage measurements for detecting autonomous thyroid tissue. Scintiscanning and recently also sonography of the thyroid are always indicated in cases of a visible and palpable enlargement of the thyroid. If palpation, scintiscanning, or sonography reveals a noticeable thyroid node a fine needle biopsy is indicated. A reasonable step-by-step diagnose should be the goal, according to the clinical questions. (orig.)

Abstract (German)

Die Diagnose von Erkrankungen der Schilddruese erfolgt durch eine gezielte Anamnese, die spezielle koerperliche Untersuchung des Patienten und den Einsatz geeigneter technischer Untersuchungsverfahren. Die einzelnen Methoden zur Abklaerung einer von der Norm abweichenden Schilddruesenfunktion sollten sorgfaeltig, je nach klinischer Fragestellung, ausgewaehlt und nicht nach einem Schema eingesetzt werden. Die Bestimmung von Thyroxin und/oder Trijodthyronin im Serum, die Messung der Bindungskapazitaet fuer Schilddruesenhormone bzw. der freien Schilddruesenhormone sowie der TRH-Test haben den 131J-Radiojod-Zweiphasentest weitgehend ersetzt, vor allem nachdem die (Gammakamera-)Szintigraphie der Schilddruese mit sup(99m)Tc-Pertechnetat oder 123J-Jodid mit Speichermessungen fuer den Nachweis autonomen Schilddruesengewebes kombiniert werden kann. Die Schilddruesenszintigraphie und - neuerdings - die Sonographie sind bei Vorhandensein einer sicht- oder tastbaren Schilddruesenvergroesserung immer angezeigt. Wenn sich bei Palpation, Szintigraphie oder Sonographie ein auffaelliger Schilddruesenknoten findet, ist die Indikation fuer eine Feinnadelpunktion gegeben. Je nach klinischer Fragestellung sollte eine sinnvolle Stufendiagnostik angestrebt werden. (orig.)
