Differentiated thyroid cancer in patients less than 20 years: Report of 20 cases

Description
Papillary thyroid cancer is rare in patients under 20 years and has a good prognosis, although it is often found at the stage of lung metastases. Our multicenter study included 20 patients aged between 5 and 18 years, female predominance, followed for differentiated thyroid cancer with lung metastases. All these patients underwent a whole body scan with I-131, serum thyroglobulin, chest radiography in 4 cases and cervicothoracic (CT) in 5 cases. Only scintigraphic imaging has shown miliary pulmonary among all patients. The therapeutic efficacy was observed in all patients after treatment with iodine-131, which proves the high sensitivity of these metastases to iodine radiotherapy. The differentiated thyroid cancer in child and adolescent is characterized by local aggressiveness and a higher frequency of distant metastases than in the adult. Prognosis does not seem to be influenced by histology alone, but by its association with other prognostic factors: age, metastases and surgery. (authors)

Le cancer papillaire de la thyroide est rare chez le sujet de moins de 20 ans et reste de bon pronostic, malgre qu'il soit souvent decouvert au stade de metastases pulmonaires. Notre etude multicentrique a porte sur 20 patients ages entre 5 et 18 ans, suivis pour cancer differencie de la thyroide. Tous les patients de cette cohorte a predominance feminine ont beneficie d'un balayage du corps entier a l'iode 131 et d'un dosage de thyroglobuline sous stimulation. Une radiographie pulmonaire a ete pratiquee dans 4 cas et une TDM cervicothoracique dans 5 cas. Seule l'imagerie scintigraphique a permis de mettre en evidence une miliaire pulmonaire chez l'ensemble des patients. L'efficacite therapeutique a ete observee chez tous les patients apres traitement par l'iode 131, ce qui prouve la bonne sensibilite de ces metastases a l'iratherapie. Le cancer differencie de la thyroide de l'enfant et de l'adolescent est caracterise par une agressivite locale et une frequence plus elevee de metastases a distance que dans la population adulte. Son pronostic ne semble pas etre influence par l'histologie seule, mais par son association aux autres facteurs pronostiques : age, metastases initiales et chirurgie. (auteurs)Additional details
