The role of radiotherapy in the management of symptomatic vertebral hemangiomas

Description

The value of radiotherapy is discussed referring to the authors' own results and a review of the literature. A total dose of 30.0 Gy applied in five weekly fractions of 2.0 Gy has proven to be effective for primary treatment and postsurgical irradiation for prevention of recurrence. The analysis of 55 studies reporting results of 210 cases with application of primary radiotherapy or combinations with other methods of treatment demonstrated that in 54% complete relief of symptoms was achieved, in 32% a partial relief, and 11% were non-responders. The analysis of 21 reports of 63 cases which were treated with radiotherapy as the sole measure of treatment revealed complete remission of symptoms in 57%, partial remission in 32%, and 11% non-responders. It is concluded that radiation therapy is very effective in the management of symptomatic vertebral hemangiomas. With regard to the delayed effects of irradiation, cases with an acute compression of the spinal cord should be treated primarily with a surgical procedure and a postsurgical irradiation is recommended to prevent a relapse of symptoms. In order to minimise acute or late toxicity and the risk of radiation-induced neoplasms, the total dose should not exceed 30 Gy. (orig.)

Abstract (German)

Die Wertigkeit der Strahlentherapie wird anhand von Fallberichten und einer Literaturuebersicht dargelegt. Im eigenen Krankengut erwies sich die konventionell fraktionierte Stehfeldbestrahlung mit 5x2,0 Gy pro Woche bis zu einer Gesamtdosis 30 Gy fuer die primaere und postoperative Bestrahlung als effektiv. Die genaue Analyse von 55 Berichten (210 Faelle) zeigte, dass durch alleinige Bestrahlung und die Kombination mit anderen Therapiemassnahmen bei 54% eine komplette Remission erzielt wurde, in 32% eine partielle und der Anteil an Therapieversagern bei 14% lag. Die Betrachtung von 63 Faellen aus 21 Arbeiten, die mittels einer alleinigen Radiotherapie behandelt wurden, erbrachte bei 57% eine komplette Remission, bei 32% eine partielle und bei 11% keine Remission. Die Strahlentherapie ist sowohl fuer die postoperative Bestrahlung, wie auch fuer die primaere Therapie eine sehr effektive Behandlungsmassnahme. Trotz der guten Resultate der alleinigen Bestrahlung sollten Faelle mit akuter Myelonkompression aufgrund der verzoegerten Effekte der Radiotherapie zur Vermeidung einer persistierenden Myelopathie primaer einer operativen Entlastung zugefuehrt werden. Zur Vermeidung von Frueh- oder Spaettoxizitaeten und zur Minimierung des Risikos der radiogenen Induktion von Sekundaermalignomen sollte eine Gesamtdosis von 30 Gy nicht ueberschritten werden.
