Contributions of imaging to sport pathologies. Synthesis of the 2020 winter conferences (January 19 to 23)
Description
There are several difficulties and pitfalls in bone scintigraphy: - technical insufficiency which can be nearly harmful; - physiological variations during the interpretation, which can mimic an anomaly; - beware of the satisfaction bias and the bias of the hypothesis (finding something does not necessarily explain the clinical picture - imputability can be doubtful, do not come to a conclusion too quickly); - watch out for incidentalomas (risk of an infernal cycle); - be careful not to overlook the negative signs, which are just as important to spot and report as the positive signs; - be careful not to omit analyzing the soft parts, which is just as important as analyzing the bone itself. Even if the clinic prevails, the examination remains poor whereas the patients are plaintiffs, from where the increase in the recourse to imagery. Interest in particular of functional imaging (MRI or OS with SPECT-CT)

Il y a quelques difficultes et pieges en scintigraphie osseuse qu'il faut bien connaitre: - attention a l'insuffisance technique: ne pas faire de temps precoce peut etre dommageable; - attention aux variations physiologiques lors de l'interpretation qui peuvent mimer une anomalie; - attention au biais de satisfaction et au biais de l'hypothese: trouver quelque chose n'explique pas forcement le tableau clinique (l'imputabilite peut etre douteuse, il ne faut pas conclure trop vite); - attention aux incidentalomes (risque de cycle infernal); - attention a ne pas negliger les signes negatifs, tout aussi importants a reperer et signaler que les signes positifs; - attention a ne pas omettre d'analyser les parties molles, ce qui est tout aussi important que l'analyse de l'os lui-meme. Meme si la clinique prime, l'examen reste pauvre alors que les patients sont demandeurs, d'ou l'augmentation du recours a l'imagerie. Interet notamment de l'imagerie fonctionnelle (IRM ou SO avec TEMP-TDM). (auteurs)Additional details
