Differential diagnosis of malignant and benign bone diseases by skeletal imaging

Description
A quantitative method was used to identify benign and melignant skeletal lesions. 57 patients were divided into four groups: (A) 29 with metastatic lesions before treatment, (B) 16 metastatic lesions during treatment, (C) 9 with primary cancer without metastasis, (D) 3 nononcologic benign lesions. Patients were injected intravenously with 740 MBq 99mTc-MDP and the radioactive concentrated lesions were investigated 4 and 24 hr after the injection and the ratio of 24hr/4hr (TF ratio) was calculated. Lesions of benign bone disease had a lower TF ratio than metastatic bone lesions. Based on ROC analysis the TF ratio 1.14 was defined as the threshold value of differential diagnosis. The sensitivity, specificity and accuracy was 64%, 79% and 68% respectively. The results showed that the quantitative method may be valuable for differentiation of metastatic from benign bone diseases
