In vivo measurement by thermoluminescence of the gamma ray radiation dose to the uterus delivered during 131I therapy of Basedow's disease
Description
131 I is often the therapy of choice for BASEDOW's disease. The determination of radiation dose to the gonads from a therapeutic dose of 131 I is therefore of importance and the accuracy of radiation dose calculation is uncertain because of the numerous biological variables involved. The dose to the uterus was directly measured in 20 volonteers with Basedow's disease using a thermoluminescent dosimeter of lithium fluoride and calcium dysprosium sulfate, attached to a copper intrauterine contraceptive device. The dosimeters were inserted at the time of administration of 131 I and were retreived one month later. By this method, the dose to the uterus from gamma rays only was measured and a gamma ray dose equal to the dose to the uterus, was assumed to the ovaries. In vivo experimental results were compared with the values calculated using the specific absorbed fractions (PHI (r2 - r1) determined by SNYDER. In the calculations, the morphology of the patient, in particular the distance from thyroid to uterus was taken into account. The in vivo measurements have also been compared with direct in vivo measurements using phantoms. In vivo measurements indicate that the average dose to the uterus and ovaries is of the order of 1 rad per 10 mCi concentrated in the thyroid gland. These figures are below the generally accepted maximum admissible dose to the gonads of 10 rems
Abstract (French)
La therapeutique a l'Iode 131 chez la femme en age de procreer est actuellement frequemment retenue cpmme therapeutique initiale en raison de ses avantages. La determination de l'irradiation des gonades par une dose therapeutique est donc un probleme important; or, les valeurs obtenues par les calculs sont souvent imprecises en raison de la multiplicite des facteurs biologiques qui entrent en ligne de compte. On a directement mesure la dose uterine chez 20 volontaires basedowiennes gace a des dosimetres thermoluminescents au fluorure de lithium et de sulfate de calcium dysprosium, ceux-ci etant fixes a un sterilet au cuivre. Ces dosimetres places au moment de l'administration de la dose therapeutique sont retires 1 mois plus tard environ. L'information recueillie concerne uniquement la dose due aux rayonnements gamma et on admet que l'irradiation uterine est equivalente a la dose delivree aux ovaires. On a compare les resultats experimentaux in vivo aux valeurs calculees en utilisant la fraction specifique absorbee (PHI(r2 - r1) determinee par SNYDER. On a tenu compte dans les calculs de la morphologie de la malade, notamment de la distance thyroide-uterus. Ces mesures in vivo ont ete egalement comparees a des mesures directes sur fantomes. Les valeurs experimentales moyennes obtenues sont de l'ordre du rad pour 10 millicuries fixees dans le corps thyroide. Les irradiations entrainees par l'administration de 8 000 a 10 000 rads au corps thyroide sont donc nettement au dessous des 10 rems de la dose maximum admissible aux gonadesAdditional details
