Breast scintigraphy as a prediction of response to neoadjuvant chemotherapy

The objectives of this work are to evaluate the usefulness of radionuclide imaging method breast as predictor of response to neoadjuvant chemotherapy, correlating it with physical examination, mammography and histopathology of the surgical specimen. Introduction: Neoadjuvant chemotherapy (preoperative or primary) is considered an essential part of the multidisciplinary treatment of locally advanced breast cancer. 99mTechnetium sestamibi enters cells by passive diffusion but through plasma and mitochondrial membranes, a process which is determined by the intensity of local blood flow. This affinity of 99mTc-MIBI by mitochondria, the large number that are present in these tissues with high metabolic and degree of angiogenesis, (abnormal neoformed vessels percentage) is the basis for application to cancer diagnosis. Patients and methods: We selected 42 patients with palpable nodules greater than 3 cm and mammogram classified as BI-RADS V. The age range spanned from 32 to 64 years, with an average of 50.33 years. All patients were performed tru-cut biopsy of breast lump. And in all of them, the pathology was positive for breast carcinoma. Also give hormone receptors, HER-2/neu and Ki-67. We asked scintigraphy with 99mTc- sestamibi breast before and after neoadjuvant chemotherapy. Was considered negative in those images where no tracer was setting, capturing images, which showed fixation, those in which the tracer binding manifested faint, and those images showed intense uptake of 99mTc. Discussion: Breast scintigraphy was not useful in discriminating patients had complete clinical response partial clinical response compared with clinical and mammography, which we were more effective in the diagnosis of residual disease. Los objetivos planteados en este trabajo son evaluar la utilidad del centellograma mamario como método imaginológico de predicción de respuesta a quimioterapia neoadyuvante, correlacionar el mismo con examen físico, mamografía e histopatología de la pieza operatoria. Introducción: La quimioterapia neoadyuvante (preoperatoria o primaria) se considera una parte esencial del tratamiento multidisciplinario del cáncer de mama localmente avanzado. El 99mTechnetium sestamibi penetra en las células por difusión pasiva pero a través de las membranas plasmática y mitocondrial, proceso que se ve determinado por la intensidad del flujo sanguíneo local. Esta afinidad del 99mTc-MIBI por las mitocondrias, el elevado número en que ellas se encuentran presentes en los tejidos con un alto metabolismo y el grado de angiogénesis (porcentaje de vasos neoformados anormales) es la base para su aplicación en el diagnóstico oncológico. Pacientes y métodos: Se seleccionaron 42 pacientes con nódulo palpable mayor a 3 cm y mamografía clasificada como BI-RADS V (ACR). El rango de edad abarcó desde 32 a 64 años, con un promedio de 50,33 años. A todas las pacientes se les realizó biopsia tru-cut del nódulo mamario. Y en todas ellas la anatomía patológica resultó positiva para carcinoma mamario. Obteniéndose también receptores hormonales, HER-2/neu y Ki-67. Se solicitó centellograma mamario con 99mTechnetium sestamibi pre y posquimioterapia neoadyuvante. Se consideró negativa en aquellas imágenes donde no se encontraba fijación del trazador; imágenes de captación, las que presentaban fijación, aquellas en las cuales la unión del trazador se manifestaba de forma tenue, así como aquellas imágenes que mostraron captación del Tc-99m intensa. Discusión: El centellograma mamario no fue útil en discriminar las pacientes que tuvieron respuesta clínica completa de respuesta clínica parcial, en comparación con la clínica y la mamografía, la cual nos resultó más eficaz en el diagnóstico de enfermedad residual.
