Diagnostic performances and therapeutic impact of fluoro-choline PET/CT: Amiens experience

Background: 18FCH PET/CT is used since March 2012 in prostate cancer at Amiens University Hospital. The purpose of this study was to evaluate diagnostic performances and therapeutic impact of 18FCH PET/CT. Methods: This retrospective and monocentric study involved 196 examinations performed at the University Hospital in Amiens between March 2012 and October 2016 in the context of prostate cancer. Validation criteria of PET and therapeutic impact were the biological monitoring of PSA levels, histopathological evidence, conventional imaging data and multidisciplinary consultation meeting reports. Results: A total of 196 PET performed in 156 patients were included in the final analysis. The mean age was 71 years, mean Gleason score was 7 and mean PSA was 28.7 ng/mL. Follow-up after PET was 13.9 months on average. PET was positive in 143 cases and negative in 20, with a sensitivity of 91% and a specificity of 64%. The PSA level was significantly different between the positive PET and negative PET groups. In patients treated for biochemical recurrence, a PSA threshold of 2.2 ng/ml was used to discriminate negative PET and positive PET. PET had a direct impact on patient therapeutic management for 98 cases (59%). Conclusion: 18FCH PET/CT has an interest particularly in the location of prostate cancer recurrences in the prostate lodge, lymph nodes or bone. It has an important impact in the therapeutic management of these patients. (author)

Introduction: La TEP/TDM a la 18FCH est utilisee depuis mars 2012 au CHU d'Amiens chez les patients atteints de cancer de la prostate. L'objectif de cette etude etait d'evaluer, dans le cadre du cancer prostatique, les performances diagnostiques et l'impact de la TEP/TDM a la 18FCH sur la prise en charge therapeutique. Materiel et methodes: Cette etude retrospective et monocentrique portait sur 196 examens realises au CHU d'Amiens entre mars 2012 et octobre 2016 dans le cadre d'un cancer de la prostate. Les criteres de validation de la TEP et de l'impact therapeutique etaient le suivi biologique du taux de PSA, une preuve anatomo-pathologique, les donnees de l'imagerie conventionnelle et les compte rendus de RCP. Resultats: Un total de 196 TEP realisees chez 156 patients ont ete incluses dans l'analyse finale. L'age moyen etait de 71 ans, le score de Gleason moyen a 7 et le PSA moyen lors de la TEP etait de 28.7 ng/mL. Le suivi apres la TEP etait de 13.9 mois en moyenne. La TEP etait positive dans 143 cas et negative dans 20, avec une sensibilite de 91% et une specificite de 64%. Le taux de PSA etait significativement different entre le groupe TEP positive et TEP negative. Chez les patients adresses pour recidive biochimique, un seuil de PSA de 2.2 ng/ml permettait de discriminer les TEP negatives et les TEP positives. La TEP avait un impact direct sur la prise en charge therapeutique des patients dans n = 98 cas (59 %). Conclusion: La TEP/TDM a la 18FCH a un interet en particulier pour la localisation des recidives de cancer de la prostate au niveau de la loge prostatique, ganglionnaire ou osseux. Elle presente un impact important dans la prise en charge therapeutique de ces patients. (auteur)
