Description
12 patients with suspected brain death were examined. In 11 of these, cerebral angioscintigraphy and bilateral angiography of the carotid artery both showed an interruption of the cerebral blood flow at the level of the base of the skull. In one patient who had been strangled, cerebral blood flow was delayed. The pathological findings of the kidney examinations were bleeding from a suprarenal rupture in one case and multiple renal infarctions in another patient. A comparison between serum samples of the potential kidney donor and the standard curve of renal sup(99m)Tc-DTPA elimination showed a reduced renal clearance in the last-mentioned patient. (orig./AK)
Abstract (German)
12 Patienten mit Verdacht auf dissoziierten Hirntod wurden untersucht. In Uebereinstimmung zwischen Hirnangioszintigraphie und beidseitiger Karotisangiographie fand sich in 11 Faellen ein Stopp der Hirndurchblutung in Hoehe der Schaedelbasis. Bei einer strangulierten Patientin war ein stark verzoegerter zerebraler Blutdurchfluss nachweisbar. Pathologische Befunde der Nierenuntersuchungen entsprachen in einem Fall einer Blutung aus einer Nebennierenruptur, bei einem anderen Patienten multiplen Niereninfarkten. Der Vergleich von Serumproben des potentiellen Nierenspenders mit der Normkurve der renalen Elimination des sup(99m)Tc-DTPA liess bei dem letztgenannten Patienten eine verminderte Nieren-Clearance erkennen.
