Influence of inaccuracy of certain parameters on dose distribution in telecobalt therapy

Description

The role of the angle of incidence on the actual distribution of doses and the inaccuracy of certain parameters recorded during preparation for the therapy or shown during successive irradiation courses are studied. Only errors which can be easily quantified are retained. These are errors concerning the irradiation apparatus (source to skin distance, angle of the rays, field dimension) and those due to factors related to the patient treated: contour measurement, entrance point of rays and positional changes of the patient. The different errors can be associated in various ways for a given therapy protocol. For the same reason, an isodose of a given nominal value will have several curves. All these tracings are lined by two curves which are called ''superior and inferior envelopes'': these border the ''incertitude area''. It can be said that at the end of the treatment, the given isodose lies in this area, it is however impossible to define its form or exact position. A few examples in the text illustrate these results and show the possible practical angles of incidence. If errors during successive irradiations are distributed at random, a certain compensation is noted and the ''areas of incertitude'' decrease. But if the errors are constantly on the same side, as is sometimes the case, the dose distribution can be very different from that predicted in the dosimetric protocol

Abstract (French)

L'objet de ce travail est l'etude de l'incidence sur la distribution reelle des doses, de l'imprecision de certains parametres enregistres lors de la preparation du traitement ou affiches au cours des seances d'irradiation successives. On n'a retenu que les erreurs facilement chiffrables et concernant, d'une part le dispositif d'irradiation (distance source-peau, duree de l'irradiation, angulation des faisceaux, dimension des champs), et d'autre part les donnees relatives au malade traite (releve des contours, point d'entree des faisceaux, variations de position du malade). Pour un protocole therapeutique defini, ces differentes erreurs peuvent se combiner de facons diverses, et une isodose de valeur nominale donnee aura plusieurs traces. L'ensemble de ces traces est enveloppe par deux courbes que l'on appelle ''enveloppe superieure et enveloppe inferieure'', la surface qu'elles limitent est la ''surface d'incertitude''. On peut affirmer qu'a la fin du traitement, l'isodose consideree se trouve dans cette surface sans pouvoir preciser ni sa forme ni sa position exacte. Quelques exemples donnes dans le texte illustrent ces resultats et montrent les incidences pratiques possibles. En fait, si l'on admet que les erreurs se distribuent au hasard lors des seances d'irradiation successives il apparait une certaine compensation et les surfaces d'incertitude diminuent. Par contre, si les erreurs se font constamment dans le meme sens et ce peut etre le cas pour certaines d'entre elles, la distribution des doses peut etre tres differente de celle prevue dans le protocole dosimetrique
