Impact of intravascular enhancement, heart rate, and calcium score on diagnostic accuracy in multislice Computed Tomography coronary angiography

Description
Purpose. To assess the effect of intravascular enhancement, heart rate, and calcium score on diagnostic accuracy in the detection of significant coronary artery stenosis using 16-row multislice computed tomography (MSCT). Materials and methods. One hundred patients (88 males; 59±11 years) with suspected coronary artery disease who had undergone conventional coronary angiography (CA) and MSCT-CA were retrospectively enrolled for the study. Patients underwent a MSCT-CA, with the following protocol: collimation 16x0.75 mm, gantry rotation time 420 ms, feed/rotation 2.8 mm, kV 120, mAs 400-500. The protocol for contrast material administration was 100 ml of Iodixanol 320 mgI/nl at 4 ml/s and the scan delay was defined with a bolus tracking technique. In all patients vascular enhancement was measured in the aortic root, and the left and the right coronary arteries. The average enhancement was used to divide the population in two groups of 50 patients each, one with lower enhancement (Low), and one with higher enhancement (High). In the two groups diagnostic accuracy (per coronary segment) for the detection of significant stenosis (≥50% lumen reduction) was evaluated in vessels ≥2mm in diameter using quantitative CA as the reference standard. The differences in diagnostic accuracy were compared with a Chi-square test and a p<0.05 was considered significant. Results. Of the total 1116 segments (≥2 mm lumen diameter), 173 presented significant stenosis. The sensitivity and specificity for the assessment of significant stenosis were 89.4% and 93.3% vs 94.3% and 97.4% in the presence of increasing intravascular enhancement, 92.8% and 96.7% vs 91.1% and 93.9% in the presence of increasing heart rate, and 89.7% and 97.6% vs 93.3% and 92.8% in the presence of increasing calcium score. Conclusions. Increasing intravascular enhancement significantly improves diagnostic accuracy in MSCT-CA. A higher heart rate lowers the specificity in the detection of significant obstructing lesions of the coronary artery. An increasing calcium score determines a lower specificity and higher sensitivity

Scopo. Valutare la variazione dell'accuratezza diagnostica dell'angiografia coronarica mediante TC multi strato (AC- TCMS) in relazione ai valori di attenuazione delle arterie coronarie, della frequenza cardiaca e del calcio coronarico. Materiale e metodi. Cento pazienti (88 maschi, eta 59±11 anni) con sospetta malattia coronarica ostruttiva e in ritmo sinusale sono stati retrospettivamente inclusi nello studio. La AC-TCMS (Sensation 16, Siemens, Germania) e stata effettuata con gating retrospettivo, dopo la somministrazione endovenosa di un bolo di 120 mI di mezzo di contrasto iodato e tecnica di bolus tracking. I parametri di scansione utilizzati sono stati i seguenti: collimazione 16xO,75 mm, tempo di rotazione 0,42 s, avanzamento/rotazione 2,8 mm, 120 kV, 500 mAs. Due osservatori hanno valutato in consenso la presenza di stenosi significative (riduzione del diametro coronarico 50%) dei segmenti coronarici con diametro 22,0 mm. Come gold-standard e stata usata la coronarografia quantitativa. La popolazione studiata di 100 pazienti e stata divisa in due gruppi con riferimento a ciascuno dei seguenti parametri: valore di attenuazione coronarica, frequenza cardiaca e calcio coronarico. In ogni gruppo e stata calcolata l'accuratezza diagnostica. Risultati. Centosettantre dei 1116 segmenti di diametro 2 mm avevano una stenosi significativa. Sensibilita e specificita diagnostica della AC- TCMS per l'individuazione delle stenosi significative sono risultate, rispettivamente, 89,4% e 93,3% contro 94,3% e 97,4% per valori crescenti dell'attenuazione coronarica, 92,8% e 96,7% contro 91,1 % e 93,9% nel caso di un aumento della frequenza cardiaca e 89,7% e 97,6% contro 93,3% e 92,8% per punteggi crescenti del Calcium score. Conclusioni. Un'elevata attenuazione coronarica (valore medio 373±41 HU) migliora l'accuratezza diagnostica della AC- TCMS. Un'elevata frequenza cardiaca (media 64±5 bpm) determina una perdita di specificita. Un elevato valore del Calcium score (media 575±758) porta ad una perdita di specificita con un concomitante incremento della sensibilitaAdditional details
