HMPAO-SPECT with diamox-stimulation test in cerebrovascular insufficiency

Besides the results of morphological non-invasive methods of investigation (TCT, NMR) in cerebrovascular insufficiency measurement in rCBF with 99mTc-HMPAO is becoming of increasing importance as a functional parameter of cerebral diagnostics. The cerebrovascular reserve capacity can be proved with acetazolamid. When considered separately for hemispheric transient ischemic attacks/prolonged reversible ischemic neurologic deficit (TIA/PRIND) with 87% and vertebrobasilar ischemia with 64%, there was a very much higher sensitivity in detection of ischemia in the provocation test with acetazolamid in normal TCT. HMPAO-SPECT with acetazolamid shows a better clinical correlation than TCT alone in reversible cerebrovascular disease. HMPAO-SPECT with acetazolamid demonstrates the hemodynamic relevance in two thirds of asymptomatic higher-degree stenoses of carotid artery and supports the indication for surgery. (orig.)
Abstract (German)
Neben den Ergebnissen morphologischer nichtinvasiver Untersuchungsverfahren (TCT, KST) bei cerebrovasculaerer Insuffizienz erlangt die Messung des rCBF mit 99mTc-HMPAO als Funktionsparameter fuer die cerebrale Diagnostik zunehmende Bedeutung. Die cerebrovasculaere Perfusionsreserve kann unter Azetazolamid nachgewiesen werden. Differenzen fuer Hemisphaeren-TIA/-PRIND mit 87% und vertebrobasilaerer Ischaemie mit 64% ergibt sich eine deutlich hoehere Sensitivitaet zum Nachweis der Ischaemie im Provokationstest mit Azetazolamid bei unauffaelliger TCT. Die HMPAO-SPECT mit Azetazolamid zeigt bei reversiblen cerebrovasculaeren Erkrankungen eine bessere klinische Korrelation als die TCT allein. Bei zwei Dritteln der asymptomatischen hoehergradigen Carotisstenosen wird mit Azetazolamid die haemodynamische Relevanz nachgewiesen und damit die Indikation zur Operation unterstuetzt.
