Differential typology of cerebral ischaemic infarctions using 99mTc-HMPAO-SPECT and clinical value in comparison to TCT
Description
The results of differential typology of cerebral ischaemic infarction as obtained by 99mTc-Hexamethylpropylene-Aminoxim (HMPAO)-Single-Photon-Emission-Computertomography (SPECT) are presented based on 41 examined patients with cerebral infarction in comparison to TCT. Macro- and micorangiopathic lesion patterns can be sufficiently differentiated by SPECT. Cerebellar diachisis usually is detected in cases of fresh territorial and stem ganglia infarctions with contralateral hemiparesis. SPECT enables reversible and irreversible tissue damage to be differentiated, thus improving both the clinical correlation and evaluation of infarction prognosis and course. (orig.)
Abstract (German)
Die Ergebnisse in der Differentialtypologie cerebraler ischaemischer Infarkte durch die 99mTc-Hexamethylopropylen-Aminoxim (HMPAO)-Single-Photonen-Emissions-Computertomographie (SPECT) werden anhand von 41 untersuchten Hirninfarktpatienten im Vergleich zur Transmissions-Computertomographie (TCT) dargestellt. Makro- und mikroangiopathische Laesionsmuster lassen sich mit der SPECT hinreichend differenzieren. Eine cerebellaere Diaschisis wird dabei in der Regel bei frischen Territorial- und Stammganglieninfarkten mit kontralateraler Hemiparese nachgewiesen. Die SPECT ermoeglicht die Unterscheidung reversibler und irreversibler Gewebeschaeden und verbessert damit sowohl die klinische Korrelation als auch die Beurteilung der Infarktprognose und des Verlaufes.
