Value of PET/CT for the management of inflammatory syndromes of undetermined origin in the Internal Medicine department of the University Hospital of Amiens

Introduction: Inflammation of unknown origin (IUO) is a common concern in the internal medicine departments. The aim of our study was to examine the diagnostic value of PET/CT for the investigation of patients with IUO in an internal medicine department. Materials and methods: We performed a retrospective cohort study in the internal medicine department of Amiens University Hospital in the patients for whom a PET/CT was performed, between October 2004 and April 2017. Patients were included if PET/CT has been prescribed for the exploration of IUO. The patients were classified according to the result: the PET/CT made the diagnosis, the PET/CT participated in the diagnosis (by targeting an area to biopsy or feeding relevantly the diagnostic argumentation), the PET/CT was negative or the PET/CT gave false positive result. Results: From the 763 patients in the cohort, 144 were included. The median age was 67.7 years [IQR55.8-75.8], with 56% men. The diagnoses finally retained were varied: 13.2% of infectious diseases, 33% of noninfectious inflammatory diseases, 16% of tumoral diseases, and 8.3% of other pathologies. However, IUO remained unexplained in 29.2% of the patients by the end of the hospital work-up with a subsequent favourable evolution in half of the cases. Fever was present in 43% of the cases, and failure to thrive in 40%. Before the prescription of PET/CT: 81% of the patients had a thoracoabdominal CT scan, 30% a colonoscopy, 43% a cardiac ultrasound examination, 27% a bone marrow biopsy, 27% a temporal artery biopsy and a 15% dental x-ray. From the 144 patients who had their PET/CT scan for IUO: the PET/CT made the diagnosis on its own in 13.2% of cases; the PET/CT participated in the diagnosis in 26% of cases; the PET/CT was negative in about 43.7% of cases. It was therefore noted that PET/CT misorient the management in 17% of situations. Contrariwise, when the PET/CT fixed and was helpful, it divided the diagnosis period between the first consultation and the date of the diagnosis by 2.7 (on average 142 days vs 63 days, p = 0.0006). And it divided by 3.1 the diagnostic delay between the date of completion of the PET/CT and the date of diagnosis (47 days vs 16 days, p = 0.007). When the PET/CT fixed (relevant result or not) it divided the median time to diagnosis after PET/CT by two (25 days [6-55] when the test fixed versus 50 days [9- 236] when it does not fix, p = 0.0323). Its median completion time was 34 days [17-85]. The result of PET/CT ended by a hypermetabolic location to biopsy in 30% of cases. In univariate analysis, PET/CT seemed to be more in patients without fever (p = 0.0467) and with general impairment (p = 0.007). When thoracoabdominopelvic CT was not contributory, PET/CT was in 21.5% of cases. Conclusion: PET/CT seems to be useful for the diagnostic workup of IUO and seems to shorten the diagnostic delay. However, its false positive results may have serious consequences. (author)

Introduction: Le syndrome inflammatoire d'origine indeterminee (SOI) est une preoccupation frequente en medecine interne. L'objectif etait d'etudier la rentabilite diagnostique de la TEP/CT pour l'exploration d'un SOI dans un service de medecine interne. Materiels et methodes: Nous avons realise une etude de cohorte retrospective dans le service de medecine interne du CHU d'Amiens chez les patients pour lesquels il avait ete realise une TEP/CT, entre Octobre 2004 et Avril 2017. Les patients etaient inclus si la TEP/CT avait ete prescrite pour l'exploration d'un SOI. Les patients etaient classes selon l'apport de la TEP/CT au diagnostic de leur SOI: l'examen a fait le diagnostic, l'examen a participe au diagnostic (en ciblant une zone a biopsier ou alimentant de facon pertinente l'argumentation diagnostique), l'examen a ete negatif ou l'examen a fait errer le diagnostic avec un faux positif. Resultats: Sur les 763 patients ayant beneficie d'une TEP/CT, 144 ont ete inclus dans l'etude. L'age median etait de 67.7 ans [IQR 55.8-75.8], avec 56% d'hommes. Les diagnostics retrouves a la fin de la prise en charge etaient varies: 13.2% de maladies infectieuses, 33% de maladies inflammatoires non infectieuses, 16% de maladies tumorales, et 8.3% d'autres pathologies. Cependant 29.2 % des etats inflammatoires restaient inexpliques a l'issue de l'hospitalisation initiale, avec une evolution ensuite favorable dans la moitie des cas. La fievre etait presente dans 43% des cas, et l'alteration de l'etat general dans 40 %. Avant la prescription de la TEP/CT: 81% des patients avaient eu un scanner thoraco-abdominal, 30% une coloscopie, 43% une echographie cardiaque, 27% une biopsie de moelle osseuse, 27% une biopsie arterielle temporale et 15% un bilan dentaire. Sur les 144 patients ayant eu leur TEP/CT pour explorer le SOI: l'examen a fait le diagnostic a lui tout seul dans 13.2% % des cas; l'examen a participe au diagnostic dans 26% des cas; l'examen etait negatif dans 43.7% des cas. Il etait donc note que la TEP/CT a fait errer la prise en charge dans environ 17% des situations. Par contre, quand elle fixait et etait rentable, elle divisait le delai diagnostique entre la premiere consultation et la date du diagnostic par 2.7 (en moyenne 142 jours vs 63 jours, p=0.0006). Et il divisait par 3.1 le delai diagnostic entre la date de realisation de la TEP/CT et la date du diagnostic (en moyenne 47 jours vs 16 jours, p=0.007). Quand la TEP/CT fixait (que l'examen soit rentable ou non), elle divisait le delais diagnostique median apres la TEP/CT par deux (25 jours [6-55] lorsque l'examen fixait, contre 50 jours [9-236] lorsque l'examen ne fixait pas, p=0.0323). Le delai de realisation median de la TEP/CT etait de 34 jours [IQR 17-85]. De facon generale, le resultat de la TEP/CT se soldait par une localisation hypermetabolique a biopsier dans 30% des cas. En analyse univariee, la TEP/CT semblait plus rentable chez les patients sans fievre (p=0.0467) et avec une alteration de l'etat general (p=0.007). Lorsque le scanner thoraco-abdomino-pelvien n'etait pas contributif, la TEP/CT l'etait dans 21.5% des cas. Conclusion: La TEP/CT garde une rentabilite correcte dans le SOI et semble raccourcir le delai diagnostique lorsqu'il fixe. Cependant, ses faux-positifs peuvent etre lourds de consequences. (auteur)
