Documentation of frailty in aged patients in radio-oncology

Background

More and more aged patients undergo radiation therapy. It is of critical importance to identify frail patients and patients with limited resources prior to initiating therapy in order to guarantee adequate supportive geriatric comanagement and/or to adapt the radio-oncological therapy concept.

Objective

Review of the evidence on geriatric screening and assessment in radiotherapy with special consideration of frailty.

Material and methods

Topic-related literature search and experiences of the authors.

Results

For aged patients (70 years and older) a geriatric screening is also recommended in radio-oncology in order to identify target patients for detailed geriatric assessment as well as needs-oriented geriatric comanagement. The evidence is inconsistent and does not allow the preference of one of the established screening instruments. Frailty screening with the Clinical Frailty Scale (CFS) was successfully implemented into the clinical routine of the initial consultation in radiation oncology after appropriate training.

Conclusion

The implementation of a frailty screening into the clinical routine is practicable. Appropriate structures for assessment and supportive geriatric comanagement should be established in order to evaluate patient reported benefits.

Zusammenfassung

Hintergrund

Zunehmend mehr betagte Patienten werden strahlentherapeutisch behandelt. Dabei ist es unverzichtbar, gebrechliche Patienten und Patienten mit limitierten Ressourcen möglichst vor Therapiebeginn zu identifizieren, um ein adäquates supportiv-geriatisches Komanagement zu initiieren und/oder auch das radioonkologische Therapiekonzept entsprechend anzupassen.

Ziel und Fragestellung

Die Arbeit diskutiert die Datenlage zum geriatrischen Screening und Assessment bei Strahlentherapie unter besonderer Berücksichtigung von Frailty (Gebrechlichkeit).

Material und Methoden

Eine themenbezogene Literaturrecherche und Erfahrungen des Autorenteams sind die Basis dieser Arbeit.

Ergebnisse

Für betagte Patienten (70 Jahre und älter) wird auch in der Radioonkologie ein geriatrisches Screening empfohlen, um die Zielpatienten für ein detailliertes geriatrisches Assessment sowie ein bedarfsorientiertes geriatrisches Komanagement zu identifizieren. Die Datenlage ist zu inkonsistent, um eines der etablierten Screeninginstrumente zu bevorzugen. Ein Frailty-Screening mit der Clinical Frailty Scale (CFS) wurde nach entsprechenden Schulungen erfolgreich in die Routine der strahlentherapeutischen Erstvorstellung implementiert.

Schlussfolgerungen

Ein Frailty-Screening ist in der Routine umsetzbar. Strukturen für das Assessment und ein supportiv-geriatrisches Komanagement sollten ergänzend hinzukommen, damit auch der patientenberichtete Nutzen evaluiert werden kann.
