In nuclear medicine it is essential to continuously optimize the diagnostic procedures applied. For this purpose, a study has been conducted with regard to the following aspects: the frequency of examinations, the various types of examinations, the indications and the frequency of pathological results (prevalence). Based on a systematic EDP-survey that has been conducted since 1979, this study also accounts for the changes observed during the past few years. Notwithstanding the marked shift from scintigraphy of the liver, spleen and brain towards alternative techniques (ultrasound, computerized tomography), there has been another increase in nuclear medical procedures since 1981. This was mainly due to increased bone and heart studies. At the same time, there was a shift in the indications for certain examinations. For example, suspicion of space occupying lesions as an indication for kidney and brain scintigraphy decreased from 40% in 1979 to 20% in 1983. In contrast, vascular diseases showed an increase to as much as 70 to 80% during the same time and thus became the most important indication for these studies. In bone scintigraphy, the number of more complex examinations (blood flow, blood pool, late static) increased from 130/1170 in 1979 to 400/2100 in 1983. Together with the significantly increased use of cardiac imaging procedures, this resulted in an increase of computer assisted examinations from 10% in 1979 to 73% in 1984. In addition, the frequency of pathological results of various types of examinations was analyzed and compared with an estimated optimum value, considering benefit, cost, risk and alternative methods. By this means important conclusions could be drawn concerning the various indications and their thresholds. Studies of this kind can serve as a basis for realistic estimates of the forthcoming requirements of nuclear medicine and thus contribute to avoid wrong investments. (orig.)

Fuer die Nuklearmedizin als methodisch orientiertes Fach ist es essentiell, ihr Untersuchungsprogramm fortlaufend zu optimieren. Hierzu erfolgte eine Analyse von Untersuchungsfrequenzen, Untersuchungsarten, Indikationen und Haeufigkeiten pathologischer Befunde (Praevalenzen), und ihrer Aenderungen in den letzten Jahren, gestuetzt auf eine seit 1979 systematische EDV-Erfassung. Trotz des deutlichen Rueckgangs der Leber-Milz- und der Hirnszintigraphie infolge alternativer Verfahren (Ultraschall, Computertomographie) kam es nach 1981 wieder zu einem Anstieg der Gesamtuntersuchungsfrequenzen. Dies war Folge einer Zunahme vorwiegend der Skelettszintigraphie und der Herzdiagnostik (Myokardszintigraphie und Radionuklidventrikulographie). Bei einigen Untersuchungsverfahren kam es gleichzeitig zu einer Verschiebung der Indikationen. Die Suche nach Raumforderungen bestand bei der Nieren- und Hirnszintigraphie 1979 in etwa 40%, 1983 nur noch in 20%. Dagegen rueckten vaskulaere Prozesse bei diesen Untersuchungen mit 70 bis 80% ganz in den Vordergrund. Auch bei der Skelettszintigraphie stieg die Zahl komplexer Untersuchungen (3-Phasen-Szintigraphie) von 130/1170 im Jahr 1979 auf 400/2100 im Jahr 1983 an. Gemeinsam mit dem starken Anstieg bei Herzuntersuchungen fuehrte dies zu einer Steigerung EDV-gestuetzter Untersuchungen von 10% an der Gesamtzahl im Jahr 1979 auf 73% im Jahr 1984. Gleichzeitig wurde die im Krankengut beobachtete Haeufigkeit pathologischer Befunde bei verschiedenen Untersuchungsarten mit einer aufgrund der Randbedingungen (Nutzen, Kosten, Risiko, alternative Verfahren) abgeschaetzten wuenschenswerten (optimalen) Haeufigkeit verglichen. Hieraus ergaben sich aufgrund des BAYESschen Theorems wichtige Hinweise auf ggf. zu niedrige oder zu hohe Indikationsschwellen. (orig.)Additional details
