Effects of angiographic installation geometry on measurement of vessel size - Implications for PTA and percutaneous placement of stents

The diameter of arteries in the angiogram is determined by the geometry of the angiographic unit and the position of the vessel in the body section. The position of the arteries in the middle portion of the pelvis is subjected to a wide variability due to their curved course, and cannot be precisely predicted even if the individual body diameter is known. Magnification of the pelvic arteries on a.p. angiograms typically ranges between 30 and 40%, and increases with patient thickness. Therefore, PTA of the pelvic arteries produces overdilatation, even if the balloon diameter is smaller than the vessel diameter measured on the a.p. film. If the balloon diameter exceeds the measured vessel diameter by 1 or 1.5 millimeters, vessel overdilatation may reach up to 70% of the original size. (orig.)
Abstract (German)
Die im Angiogramm ausgemessene Gefaessweite wird wesentlich durch die Projektionsgeometrie der verwendeten Angiographieanlage und die Lage des jeweiligen Gefaessabschnittes innerhalb des Patientenkoerpers bestimmt. Die Lage der Gefaesse im mittleren Beckenabschnitt unterliegt wegen des bogigen Gefaessverlaufes besonders grossen individuellen Unterschieden und ist daher auch in Kenntnis des Koerperdurchmessers schlecht vorhersehbar. Bei a.p. Projektion sind Vergroesserungen der Beckengefaesse von 30-40% typisch. Die Vergroesserung der Gefaesse nimmt mit der Dicke des Patienten zu. Bei der PTA der Beckenarterien erfolgt selbst dann eine Ueberdilatation, wenn der Ballondurchmesser kleiner als die Gefaessweite im a.p. Blattfilmangiogramm gewaehlt wird. Ballondurchmesser, die die ausgemessene Gefaessweite um 1 bis 1,5 mm ueberschreiten, fuehren im Einzelfall zu Ueberdilatationen von bis zu 70%. (orig.)Additional details
