A case of pseudoaneurysm bleeding at salvage surgery after chemo-radiation therapy for hypopharyngeal cancer

Description

With advances in drug therapy, the indications for chemo-radiation therapy (CRT) for head and neck carcinoma have expanded, and at the same time, the frequency of salvage surgery at recurrence has increased. Complications after salvage surgery include wound infection, fistula, and wound dehiscence, sometimes leading to fatal complications, such as total flap necrosis and carotid artery rupture, which are generally more frequent than in cases of initial surgery. We herein report a case of massive bleeding from a pseudoaneurysm in a branch of the lingual artery as a postoperative complication of salvage surgery after CRT. Previous reports have suggested a history of irradiation, surgery, and inflammation associated with ulceration as potential causes of pseudoaneurysm formation, which should be kept in mind as a possible postoperative complication in patients with such histories. Abstract (Japanese)

薬物治療開発の進歩に伴い、頭頸部癌に対する化学放射線療法(chemo-radiation therapy : CRT)の適応は拡大し、同時に再発時の救済手術の頻度も増加している。救済手術後の合併症の多くは創傷感染、痩孔、創部離開などで、時には皮弁全壊死や頸動脈破裂などの致死的合併症に至ることもあり、初回手術例よりもそれらの頻度は一般的に高い。今回われわれはCRT後の救済手術の術後合併症として、舌動脈分枝に発生した仮性動脈瘤からの大量出血に難渋した症例を経験した。過去の報告から、仮性動脈瘤形成の原因として、放射線照射、外科的手術の既往や、潰瘍に伴う炎症の既往が挙げられ、これらの既往を持つ症例の起こり得る術後合併症として、あらかじめ念頭に置く必要があると思われた。(著者)
