Is it necessary to check the homogeneity of iridium 192 wires

Description

Several aspects are involved in a brachytherapy quality assurance program including verification of computerized dose distribution, source calibration, uniformity of linear intensity along the source, tests to detect any radioactive leaks, etc. Our study is specifically confined to the verification of homogeneity and calibration of iridium 192 radioactive sources. After defining, H, as inhomogeneity and discussing the advantages, disadvantages and limitations of the different methods to derive it, the authors present the results obtained between January 1989 and March 1991 by checking 640 iridium wires from each of two European manufacturers (Cis Bio-International and Amersham International). Approximately 2% of the wires examined were found to have an heterogeneity greater than 10%, and in 8% of the wires a 3% discrepancy was detected between the calibration performed by the manufacturers and our own measurements. This observation indicates that source verification is mandatory. The effects of inhomogeneity on dose specification is analysed. The homogeneity criteria used in our department for quality control in clinical applications are presented; they take into account the absolute value of inhomogeneity as well as their length

Abstract (French)

L'assurance de qualite en curietherapie comprend entre autre le controle des programmes de calcul de distributions de dose, l'etalonnage des sources, le controle de l'uniformite de l'activite, les tests de contamination... Notre etude s'interesse plus particulierement au controle de l'homogeneite et a l'etalonnage des sources continues d'iridium 192. Apres avoir rappele la definition de l'heterogeneite, H, d'une source et discute les avantages, inconvenients et limitations des differents moyens utilisables pour la determiner, les auteurs presentent les resultats qu'ils ont obtenus de janvier 1989 a mars 1991 en verifiant 640 fils provenant alternativement de deux fournisseurs europeens differents (CIS-Bio-International et Amersham International plc). De l'ordre de 2% des fils presentaient une heterogeneite superieure a 10%, et 8% d'entre eux un ecart d'etalonnage superieur a 3% par rapport a la valeur indiquee par les fabricants. Ceci montre que la verification des fils est indispensable. L'incidence que peuvent avoir les heterogeneites sur la specification de la dose est ensuite etudiee. Des criteres d'homogeneite utilises dans le service pour garantir des applications cliniques de bonne qualite sont presentes.
