Background: Magnetic Nuclear Resonance Cholangiography (CRMN) is a diagnostic method that non-invasively evaluates the bile duct and pancreatic duct, synthesizing most of the advantages of invasive and non-invasive radiological studies. Objectives: To analyze the experience with the use of the CRMN, evaluating its usefulness and its application in the study of biliopancreatic pathology. Method: The 114 studies requested and / or controlled by doctors from the General Surgery service during the period between August 1996 and March 1999 were analyzed. Studies carried out in other institutions were not included for their study. The correlation of the CRMN images with the contrasted studies, the intraoperative findings, and the clinical evolution of the patients were analyzed. Results: 54 patients with a presumptive diagnosis of choledochal lithiasis were evaluated. The CRMN had a sensitivity of 93.5 percent, specificity of 95.6 percent, PPV of 96.6 percent, NPV of 91.6 percent and a certainty index of 0.94. In 39 patients with benign and malignant strictures, the sensitivity was 95.0 percent, the specificity was 88.8 percent, a PPV of 97.4 percent, NPV of 80.0 percent, with a certainty index of 0, 93. Evaluated globally, the CRMN presented a sensitivity of 94.7 percent, a specificity of 94.4 percent, a PPV of 97.2 percent, NPV of 89.4 percent and a certainty index of 0, 94. Conclusions: The CRMN is a non-invasive diagnostic method, with high rates of diagnostic certainty, which makes it a valid alternative to invasive procedures, displacing ERCP and percutaneous cholangiography in those cases in which immediate treatment is not proposed. by these routes.

Antecedentes: La Colangiografía por Resonancia Nuclear Magnética (CRMN) es un método diagnóstico que evalúa de manera no invasiva la vía biliar y el conducto pancreático, sintetizando la mayoría de las ventajas de los estudios radiológicos invasivos y no invasivos. Objetivos: Analizar la experiencia con el uso de la CRMN, evaluando su utilidad y su aplicación en el estudio de la patología biliopancreática. Método: Se analizaron los 114 estudios pedidos y/o controlados por médicos del servicio de Cirugía General durante el período comprendido entre agosto de 1996 y marzo de 1999. Los estudios realizados en otras instituciones no fueron incluidos para su estudio. Se analizó la correlación de las imágenes de CRMN con los estudios contrastados, los hallazgos intraoperatorios, y la evolución clínica de los pacientes. Resultados: Fueron evaluados 54 pacientes con diagnóstico presuntivo de litiasis coledociana. La CRMN tuvo una sensibilidad del 93,5 por ciento, especificidad de 95,6 por ciento, VPP de 96,6 por ciento, VPN de 91,6 por ciento y un índice de certeza de 0,94. En 39 pacientes con estenosis benignas y malignas la sensibilidad fue 95,0 por ciento, la especificidad de 88,8 por ciento, un VPP de 97,4 por ciento, VPN de 80,0 por ciento, con un índice de certeza de 0,93. Evaluada en forma global, la CRMN presentó una sensibilidad de 94,7 por ciento, una especificidad de 94,4 por ciento, un VPP de 97,2 por ciento, VPN de 89,4 por ciento y un índice de certeza de 0,94. Conclusiones: La CRMN es un método de diagnóstico no invasivo, con elevados índices de certeza diagnóstica, que la convierte en una alternativa válida frente a procedimientos invasivos, desplazando a la CPRE y colangiografía percutánea en aquellos casos en los que no se plantea un tratamiento inmediato por dichas vías.
