HPES methodology for nuclear power plant event root cause investigation
Description
Event root cause investigations are of great importance for safety operation of nuclear power plants. The author briefly describes how to use human performance enhancement system and event investigation techniques to evaluate equipment and human performance problems, uncover root causes, and determine corrective actions to prevent recurrence. The author also proposes a general process for event root cause investigations with an example attached to illustrate it
