Interest and prospects of terbium-161 for Targeted Radionuclide Therapy: set-up in Bordeaux

Targeted radionuclide therapy (TRT) of second generation (using somatostatin analogues or PSMA inhibitors radiolabeled with lutetium-177) is getting more and more importance in oncological therapeutic strategies. However, further optimizations are required to improve survival of patients with metastatic cancers. Terbium-161 (161Tb) demonstrates similar chemical properties with lutetium-177, with much higher proportion of low energy electrons emissions (Auger, Intern Conversion, Coster-Kronig), enabling increased dose-deposits in single tumor cells and micrometastases. Considering the rising interest of 161Tb in Europe, this work describes the preliminary steps before the set-up of this radionuclide in Bordeaux, in particular regulatory authorizations for detention and use, exclusion chromatography purification method of 161Tb (first with non-radioactive terbium) and then (radio)labeling. Abstract (French)
La radiotherapie interne vectorisee (RIV) de deuxieme generation (analogues de la somatostatine ou inhibiteurs du PSMA radiomarques au lutetium-177) prend de plus en plus de place dans les strategies therapeutiques en oncologie. Cependant des optimisations restent necessaires pour ameliorer la survie de ces patients metastatiques. Le terbium-161 (161Tb) presente des proprietes chimiques similaires au lutetium-177, tout en emettant une part plus importante d'electrons de faible energie (Auger, Conversion Interne, Coster-Kronig) responsables d'une meilleure irradiation de cellules tumorales isolees et micrometastases. En raison de l'interet croissant du 161Tb en Europe, ce travail decrit les etapes prealables necessaires a l'implantation de ce radio-isotope innovant a Bordeaux notamment les aspects reglementaires relatifs au regime d'autorisation de detention et utilisation, methodologie de purification du 161Tb par chromatographie d'exclusion (d'abord a l'aide du terbium non radioactif) et realisation de (radio)marquage.
