Significance of the new generation of double monoclonal antibody assays for CEA

A new generation of double monoclonal antibody assays (RIA) and (EIA) for CEA was compared to a well established double polyclonal assays system (RIA and EIA). The monoclonal assays revealed a better handling and could be performed in a shorter time. Sensitivity and specificity of all four assays were analysed by means of cumulative distribution- and receiver-operator-characteristic-curves. There were no differences in discriminating power between healthy individuals and patients with tumours. Comparison of CEA concentrations in a retrospective follow-up study of two patients showed no clinically important deviation in the follow-up curves determined by mono- and polyclonal RIA. As a conclusion the investigation of the new double monoclonal immunoassays results in increased practicability without loss of discriminating power for cancer patients. (orig.)
Abstract (German)
Eine neue Generation von CEA-Tests (Abbott-RIA-monoklonal, Abbott-EIA-monoklonal) mit CEA-Antikoerpern monoklonaler Herkunft wurde mit CEA-Tests verglichen, bei denen konventionell hergestellte polyklonale CEA-Antikoerper im Testbesteck enthalten waren (Abbott-RIA-polyklonal, Abbott-EIA-polyklonal). Bei den Tests mit Antikoerpern monoklonaler Herkunft konnte eine verbesserte Handhabung in der Testdurchfuehrung und eine Verkuerzung der Antikoerperinkubationszeiten festgestellt werden. Der Vergleich der Sensitivitaet und Spezifitaet anhand von Verteilungs- und ROC-Kurven zeigte gleiche diagnostische Wertigkeit fuer alle untersuchten CEA-Tests, wenn gesunde Personen mit Tumorpatienten verglichen wurden. Bei der Untersuchung des klinischen Verlaufs wurden ebenfalls keine Unterschiede zwischen den CEA-Verlaufskurven der einzelnen Tests festgestellt. Somit zeigen die neuen monoklonalen CEA-Tests von Abbott praktische Vorteile bei gleicher diagnostischer Sensitivitaet. (orig.)Additional details
