Highlights: • MpMRI of prostate could detect the majority of index PCa and csPCa. • ADC correlated better with GS and cellularity metrics than T2 within lesions in PZ. • ADCmean performed better than ADC10% in aggressiveness evaluation of PCa. • Both ADC and T2 parameters demonstrated poor performance within lesions in TZ. -- Abstract: To compare the diagnostic performance of multiparametric MRI (mpMRI) parameters for Gleason score (GS) and cellularity metrics of prostate cancer (PCa) in the peripheral zone (PZ) and transition zone (TZ) separately.
