Fetal dose with the 'Mantle ' techniques in radiotherapy

Description

A patient in the 26th. week of pregnancy with Hodgkin's Disease is studied. A variation of dose in points outside the irradiation field for different distances in Mantle techniques was simulated, with and without special shielding around the patient's abdomen. A dose of 2.250 rad was firstly delivered, 250 rad four times each week, using parallel opposing portals at source-surface distance 80 cm. The dose to the head, gonad and fetus was calculated in 20.2, 10.1 and 15.8 rad respectively. A male child was born at the 34th. week by normal delivery; subsequently, the dose was brought to 4.400 rad. Six months after treatment the patient is assyntomatic and the child is psychosomatically normal

Abstract (Portuguese)

Estuda-se um paciente, na 26a. semana de gravidez, portador de Doenca de Hodgkin. Estudou-se a dose a ser recebida pelo feto durante o tratamento com tecnica de 'Mantle' modificada. Decidiu-se pela irradiacao com distancia fonte-superficie de 80.0 cm, pela impossibilidade de a paciente deitar-se em dicubito ventral. Numa primeira fase fizeram-se 2.250 rad em 9 fracoes de 250 rad cada, em campos paralelos e opostos, 4 fracoes por semana com 1 campo por dia. Desse modo estimou-se a dose na cabeca, em 20.2 rad, na gonada fetal de 10.1 rad e no corpo todo fetal de 15.2 rad. Apos 25 dias do termino dessa fase nasceu de parto normal na 34a. semana de gestacao uma crianca do sexo masculino. Posteriormente, a dose foi elevada para 4.400 rad. Atualmente, apos 6 meses de tratamento, a paciente esta assintomatica e a crianca em desenvolvimento psicossomatico normal
