Highlights: • Symptomatic plaques have higher enhancement and intraplaque hemorrhage presence. • Intracranial plaque enhancement was associated with the ischemic event. • Vessel wall imaging can identify high-risk plaque without substantial stenosis. • High-resolution black-blood magnetic resonance imaging can stratify stroke risk. To characterise symptomatic intracranial plaque without substantial stenosis using three-dimensional (3D) high-resolution contrast-enhanced black-blood magnetic resonance imaging (BBMRI) and to determine plaque features associated with recent cerebrovascular events.
