Assessment of diastolic properties of left venticle by Fourier analysis of global left ventricular volume curve obtained from RI angiography

Description

To assess the resting left ventricular diastolic preperties in the patients with ischemic heart disease, we analyzed global left ventricular volume curve obtained by radionuclide angiocardiography with Fourier analysis in 72 patients. Analysis in 22 patients revealed that 4 harmonics approximation in the Fourier series could be within Poisson noise. Correlation coefficient between original volume curve and 2 harmonics approximation was 0.987. We made a resurge curve with the value of first harmonic longitudinally and 2 harmonics approximation horizontally. Diastolic phase pattern of the resurge curve was S shape in group-1 (EF gt = 60%), straight line in group-2 (40% lt = EF lt = 60%) and reversed S shape in group-3 (EF lt 40%). DE, expressed as tangential slope at mid-point of the early half diastolic phase, was 0.65 +- 0.13 (mean +- SD), 0.99 +- 0.28 and 1.33 +- 0.39 (group-1, group-2 and group-3, respectively). DL, expressed as the tangential slope at mid-point of the late half diastolic phase, was 2.04 +- 0.78, 1.11 +- 0.32 and 0.81 +- 0.18. DL/DE was 3.40 +- 1.90, 1,25 +- 0.61 and 0.64 +- 0.19. These data indicates that the left ventricular diastolic filling was mainly in the early half diastolic phase in the patients with normal left ventricular function, and that the amount of diastolic filling in the atrial contraction phase increased in the patients with reduced left ventricular function. It is concluded that DE, DL and DL/DE measured from the resurge curve made by Fourier analysis of radionuclide angiocardiographic left ventricular volume curve is a useful method to evaluate the left ventricular diastolic properties.
