Immunoassay of the free fraction of T4. Comparative study of three nonisotopic and two two-step isotopic immunoassays

Description
Radioimmunoassays (RIA) of the free fraction of thyroxin using labeled analogs give artefacts in certain disorders now well known. Three nonisotopic (Amerlite, Magic Lite, Stratus) and two two-step isotopic immunoassays (Berhing and Clinical Assays), are studied by comparison with two FT4 Radioimmunoassays: Amerlex and Sclavo. With the all methods, results were similar to those obtained by the Sclavo assay: intra and inter assays coefficients of variation less than 10% in the working range and no significant interaction between tracer and thyroxin binding protein. In a population of 325 euthyroid subjects, 111 hyperthyroid and 61 hypothyroid patients without any treatment, slight overlaps were observed between the hyperthyroid and euthyroid populations and diagnostic sensitivities were for hyperthyroidism and hypothyroidism about 0.95 and 0.80. Results were comparable to those obtained with the Sclavo assay in specific populations in which FT4 concentrations are known not to be always related to their euthyroid status (69 patients with chronic renal failure, 20 patients in intensive-care, 17 patients treated with heparin, and 27 pregnant women). The methods studied gave globally good results and were better than the one-step analog based method
Abstract (French)
Les dosages radioimmunologiques (RIA) de la fraction libre de la thyroxine a l'aide de traceurs analogues encore largement utilises aujourd'hui sont perturbes par des interactions traceur-proteine vectrice dependantes du contexte pathologique extra-thyroidien. Les methodes utilisant des traceurs conjugues et/ou faisant appel a deux etapes ne presentent theoriquement pas ces inconvenients. Les performances de trois methodes non isotopiques (Amerlite, Magic Lite, Stratus) et de deux RIA (Berhing et Clinical Assays) recemment commercialisees sont comparees dans ce travail a celles de la methode RIA Amerlex (une etape analogue) et celles de la methode Sclavo (deux etapes avec chromatographie). Les resultats analytiques obtenus avec les 5 trousses testees sont tres proches de ceux de la methode deux etapes Sclavo (CV inferieur a 10% - pas d'interaction traceur-proteines vectrices). 325 sujets euthyroidiens, 111 hyperthyroidiens et 61 hypothyroidiens, tous sans traitement ont ete etudies. Le recouvrement eu-hyper est tres faible et les sensibilites diagnostiques pour l'hyper et l'hypothyroidie sont respectivement aux environs de 0,95 et 0,80. Les resultats obtenus chez quatre populations particulieres d'euthyroidiens (69 hemodialyses, 20 sujets de reanimation, 17 patients sous heparine et 27 femmes au 3eme trimestre de grossesse) confirment les qualites de ces cinq trousses qui ne presentent pas les inconvenients des methodes par analogueAdditional details
